B1/D6/D9/E. The entity listed on B1 must match the person/firm who is responsible for land -disturbing activity listed on
the FRO form, NOI Certification form (uploaded to D9), and Tab E (certification tab). You will need to contact the Raleigh
Regional Office to have your FRO form revised to reflect the correct responsible entity, as shown on your NOI
certification form and eNOI.
