Dear Ms. Rivers:
Ron Boone, of the NC Division of Water Quality(DWQ),Winston Salem Regional Office (WSRO),
conducted a compliance evaluation inspection of your single family residence wastewater treatment
system located at 4669 Paula Drive, in Winston-Salem, Forsyth County, NC, on January 12, 2012. The
chlorinator for your system could not be located during that inspection. Mr. Boone has tried several times
since to contact you and resolve this issue, but has been unsuccessful in those attempts.
Please note that the absence of the chlorinator and the failure to chlorinate the wastewater before
it is discharged to the stream are violations of the NCG550000 general permit and NC General Statute
143-215.1. Such violations are subject to civil penalty assessments not to exceed $25,000 per day per
violation.
Please contact Mr. Boone at your earliest convenience to update him on your progress to locate
the chlorinator and begin using it as intended, or to install a new chlorinator. Your failure to do so may
result in this office initiating civil penalty proceedings.
