However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on November 2, 2023. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. Cover Letter: 1. No comments. B. Application Fee: 1. No comments. C. Application (Form: SFRWWIS 06-16): The application lists the facility address as 4207 HWY 801 N, Mocksville North Carolina. According to GIS data, however, the parcel described by the plans (Parcel No. 5823651469) is located at 24752 Kates Dr., Albemarle North Carolina. Please confirm where the facility is to be constructed and correct any references of the address found throughout the application package. D. Existing Permit: 1. Not applicable. E. Property Ownership Documentation: 1. The property deed included in the permit package has been written in long hand cursive and was mirrored during the scanning process. This document is illegible in its current state. It cannot be determined whether the deed provided corresponds to the property listed. Please provide the most recent deed to the property. F. Soil Evaluation: 1. A 48% drainage coefficient is being proposed. Generally, a maximum drainage coefficient of 10% is adhered to for design purposes. Regional Staff also noted that a similar soil series in the region utilized a drainage coefficient of 8%. Please explain the reasoning for utilizing such a large drainage coefficient in the design. 2. Per 15A NCAC 02T .0604(b)(2), Ksat values are to be based upon the most restrictive horizon. The soils report, however, states that the Ksat has been measured in the "least restrictive horizon". Please clarify and or update the soils report. Additionally, regional staff noted that the last reading of each nest was higher than previous measurements. Upon further investigation, a general trend upward over the course of the last three measurements of Nests 2 and 3 was observed. Please explain how steady state was determined. Application form SFRWWIS 06-16 requires all soil boring logs to be provided within the soil evaluation. Please provide the soil boring logs for each boring that was performed. 4. According to the summary table for the soil borings, all measurements of the seasonal high water table were found to be less than 18" from the ground surface and no mention of a perched water table was made. Per 15A NCAC 02T .0605(p), a minimum of 18" of vertical separation shall be Ms. Darla Smith October 2, 2023 Page 3 of 4 provided. Please explain how this separation distance will be maintained and document any required fill within the plans and soils report. 5. Regional staff made note that the intended cover crop found in the soil evaluation consisted of "mixed grasses". Form: SFRWWIS 06-16, however, lists the cover crop to be "mature forest". Please clarify which cover crop is to be used and ensure that agronomic rates will not be exceeded. G. Engineering Plans: 1. Form: SFRWWIS 06-16 requires the wetted area of emitters and the location of air releases to be illustrated within the site plans. Please include these details within the drawings. H. Specifications: 1. No comments. I. Engineering Calculations: 1. According to Section 2 of the pump sizing calculations, the pressure at the beginning of the drip field is nearly half the required pressure calculated for the field. Please revise this calculation to ensure conveyance throughout the lines. J. Site May: 1. No comprehensive site map was included in the permit application. Please submit a site map that includes illustrations of all setbacks as stipulated by 15A NCAC 02T .0606. Please also ensure any drinking water wells associated with the residence have been included on all maps/plans with associated buffers. 2. Regional Staff noted the existence of a ditch to the east of the irrigation field. Please ensure that this surface water diversion feature has been buffered and accounted for. K.
