B1/D6/D9/E. The entity listed on B1 must match the person/firm who is responsible for land -disturbing activity listed on
the FRO form, NOI Certification form (uploaded to D9), and Tab E (certification tab).
The letter of approval should be addressed to the responsible entity listed on the project's FRO form. That same
responsible entity should be listed on the NOI certification form (Tab D9), and the NCG01 application's Tab B1 and Tab E.
There should only be one responsible entity listed for the project and it should match across the NCG01 application and
supporting documentation.
Please submit a new NOL To auto -populate the new NOI, select'Yes' for the first question & enter NOI number 177981.
