Director Randy Grogan 3585 Greeson Country Rd Climax, NC 27233 NORTH CAROLINA Environmental Quality January 11, 2024 SUBJECT: Compliance Evaluation Inspection Report 3585 Greeson Country Road NPDES WW Permit No. NCG550120 Randolph County Dear Mr. Grogan: On December 15, 2023, Division of Water Resources (DWR) Winston-Salem Regional Office (WSRO) staff Jesse Barnes conducted a routine compliance evaluation inspection of the single family residence wastewater treatment system located at 3585 Greeson Country Rd, Climax, NC 27233, in reference to NPDES general permit NCG550000 and certificate of coverage NCG550120. Mr. James Grogan, owner, was present during the inspection. A review of the subject permit, records, treatment components, outfall area, operations, and maintenance was completed. This review reflected noncompliance with the subject permit. The determined permit violations and items of concern are listed below. Inspection findings are detailed in the attached Water Compliance Inspection Report. The following violation(s) were noted during the inspection: Inspection Area Description of Violation Effluent Sampling Effluent monitoring has not been conducted. Part I, Section C. (I.) of the subject permit requires the discharge to be monitored and analyzed annually for the listed parameters. In addition, the following item(s) of concern were noted during the inspection: Inspection Area Item of Concern Recordkeeping No records for septic tank pumping were available during the inspection. The indicated permit violation(s) must be corrected to achieve compliance with the subject permit. Measures should be taken to address the indicated item(s) of concern. Please be aware that the Division may take enforcement action against any individual who fails to comply with the requirements of all applicable permits, regulations, and/or statutes. (1.) of the subject permit requires the discharge to be monitored and analyzed annually for the listed parameters. Item(s) of Concern 2. No records for septic tank pumping were available during the inspection. Other observations: Mr. Grogan stated that there have not been any problems with the system since the previous inspection. Two chlorine tablet tubes were observed within a well on the southeastern side of the home. The chlorine tubes contained tablets appropriate for domestic wastewater disinfection (70% calcium hypochlorite) and appeared to be operating properly during the inspection. Mr. Grogan stated that a new septic tank was installed approximately three years ago. He also stated that the septic tank is pumped once every three years and was last pumped approximately 1.5 years ago, but no records were available. Mr. Grogan stated he has located the outfall pipe but that it is not safely accessible due to thick vegetation and uneven terrain, so the outfall and receiving stream were not inspected.
