The proiect area comprises the future corridor of the Surf Citv Bridge replacement proiect. This new
NC 50/210 from the mainland side of the Waterwav to the island side of Tousail Island in Surf Citv.
Description of projects arca and activity: To properly design the future bridge, the applicant needs to investigate the underlying
�eolo�y within the brid�e corridor and this will involve 58 Standard Penetration Test (SPT) borings. These borings will be 3-8
inch diameter borings in both water, wetlands, and shallow sub-tidal areas and the disturbance will not extend more than a 2
foot radius around each borin� location.

Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the attached
conditions and your submitted application and attached information dated September 14, 2015. Any violation of the attached
conditions or deviation from your submitted plans may subject the permittee to a stop work order, a restoration order, a Class
I administrative penalty, and/or appropriate legal action.
This verifieation will remain valid until the expiration date identified below unless the nationwide authorization is modified,
suspended or revoked. If, prior to the expiration date identified below, the nationwide permit authorization is reissued and/or
modified, this verification will remain valid until the expiration date identified below, provided it complies with all requirements of
the modified narionwide permit. If the nationwide permit authorizarion expires or is suspended, rcvoked, or is modified, such that the
activity would no longer comply with the terms and conditions of the narionwide permit, acrivities which have commenced (i.e., are
under construction) or are under contract to commence in reliance upon the nationwide permit, will remain authorized provided the
activity is completed within twelve months of the date of the nationwide permit's expiration, modification or revocation, unless
discrerionary authority has been exercised on a case-by-case basis to modify, suspend ar revoke the authorization.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification. You
should contact the NC Division of Water Quality (telephone 919-807-6300) to determine Section 401 requirements.
For activities occurring within the twenty coastal counties subject to regulation under the Coastal Area Management Act (CAMA),
prior to beginning work you must contact the N.C. Division of Coastal Management in Morehead City, NC, at (252) 808-2808.
This Department of the Army verification does not relieve the permittee of the responsibility to obtain any other required Federal,
State or local approvals/permits.

Determination of Jurisdiction:
A. � Based on preliminary information, there appear to be waters of the US including wetlands within the above described project
area. This preliminary determination is not an appealable action under the Regulatory Program Administrative Appeal Process
( Reference 33 CFR Part 331).
B. ❑ There are Navigable Waters of the United States within the abovc dcscribed projcct area subject to the permit requirements of
Section 10 of the Rivers and Harbors Act and Section 404 of the Clcan Water Act. Unlcss therc is a changc in the law or our
published regulations, this determination may be rclied upon for a period not to exceed five years from thc datc of this
notification.
C. ❑ There are waters of the US and/or wetlands within the above dcscribed project area subject to the permit requirements of
Section 404 of the Clean Water Act (CWA)(33 USC § 1344). Unlcss there is a change in thc law or our published regulations,
this dctermination may be relied upon for a period not to excecd five years from the date of this notification.
D. ❑ The jurisdictional areas within the above deseribed project area have been identified under a previous action. Please reference
jurisdictional determination issued . Action ID: SAW- .
BaS1S FOr DeteYminatlOn: The subject areas fall within either an area characterized by wetlands or below the
ordinary high water mark. All wetlands were classified using the 1987 Corps Delineation Manual and its appropriate regional
supplement.
Remarks: Previous Preliminary Jurisdictional Determination completed on this project corridor.

Test Bore Exploration
1. Particular caution should be exercised during the moving of the rigs between sub-tidal areas
and the adjacent marshes and wetlands. This should include limiting movements during low tide
events to reduce the amount of suspended sediments released, as practicable. Shifting and turning
movements should be minimized to avoid any unnecessary disturbance to exiting marsh and root
mats. Any rutting as a result of the rig movement should be repaired and grade restored upon
completion.
2. A commitment was made to complete the work between October and January, which is
outside any moratorium windows. If the work is not completed within this time frame the
applicant needs to contact this Corps Office to determine if additional consultation with resource
agencies is necessary.
