I am sending this email per your request, to summarize the root cause and corrective actions associated with the late Sept
2023 eDMR submission.
In September a new plant manager was assigned to the Henderson facility. Since he had worked in that capacity
previously in Henderson his account needed to be reactivated to certify eDMR's. There was an issue with the permit that
his account was reactivated under and that delayed the transfer. During the gap there was an inaccurate presumption that
the previous plant manager was continuing certifications. The non-compliance was identified by Ardagh staff, the current
plant manager's account was modified and all eDMR's have been certified. As we discussed, the Oct 2023 eDMR was
also certified late.
To prevent a recurrence an internal notification process of eDMR submission and certification has been established and is
being monitored on a regular basis.
Please note that there were no discharges during the period of late certifications.
