FACILITY SUMMARY
Countryside NC MEP Sophia NC, LLC. operates a 100% domestic wastewater treatment plant for an

RENEWAL SUMMARY
This renewal contains the following changes:
• Updated the Supplement to Cover Sheet with the specific treatment components that were
installed/constructed at the wastewater treatment plant and removed the previous system
components that are no longer accurate.
• Updated eDMR language for electronic data submission in Part L A. (2).
• Updated the facility grade from WW-2 to WW-1 in Part I.
COMPLIANCE HISTORY
Countryside NC MHP has five enforcements on record for fecal coliform limit violations in 2022 and fecal
coliform and BOD limit violations and a reporting violation in 2023. The facility has been issued five
NOVs for BOD, fecal coliform, total ammonia nitrogen (as N) — concentration, and pH limit violations in
2019, 2021, and 2022. This facility has been issued six NODS for BOD limit violations in 2019 and 2020
and BOD, fecal coliform, total ammonia nitrogen (as N) — concentration, and total suspended solids
monitoring violations in 2022. The facility has also been issued 11 proceed to enforcement cases for BOD
and fecal coliform limit violations in 2022 and 2023. Most recent inspection (August 2023) showed the
facility to be in compliance.
