Upon arrival I spoke with the homeowner and she made me aware that the last time she had her septic tank pumped out it was 2009
and that the inspector came and told her that she would need to have the septic tank looked at. The homeowner then showed me the
location of the septic tank. I proceeded to do an analysis and came to the conclusion that it needed to be pumped. I then gave the
homeowner multiple upfront options and they decided on the Pump Special Plus option. I proceeded to pump and agitate the septic
tank. Whilst pumping I noticed dirt on the bottom of the tank and made the homeowner aware that I'd like to get our sewer to come
out and inspect the septic tank further. She accepted and I scheduled the inspection. Once complete I cleaned up all tools and
materials.

I have reviewed the estimated cost of E for the proposed work to be completed for this job by Quality Service Today, U.C. I agree and
acknowledge that this amount is a good faith estimate by Quality Service Today, LLC based on Quality Service Today, U.C's understanding of the
work to be completed, and that the actual cost may be more expensive, even substantially more expensive, than this estimated cost. factors that
could cause the estimated cost to rise include, but are not limited to, additional materials necessary, unforeseen problems, additional necessary
labor or materials, and other circumstances. l agree and acknowledge that Quality Service Today, LLC will take reasonable steps to keep me
informed about any price increases, but that Quality Service Today, LLC is not obligated to keep me informed about any price increases. I agree
and acknowledge that Quality Service Today, LLC does not provide a time and materials breakdown for the work it performs. I agree and
acknowledge that Quality Service Today, LLC will only perform the work Identified herein and no other work absent a signed request from
myself. I agree and acknowledge that the work is subject to Quality Service Today, LLC's terms and conditions, which are incorporated herein. By
my signature, I agree to authorize Quality Service Today, LLC to begin work pursuant to this agreement.
You, the buyer, may cancel this transaction at any time prior to midnight of the third business day after the date of this transaction. See the
attached Notice of Cancellation form for an explanation of this right.
Terms & and Conditions:
1. Warranties and Exclusion of Warranties.
A. Quality Service Today, LLC warrants the parts and labor provided by it for a period of

1 year. The 1 year warranty period begins when the work is complete as evidenced
by an invoice issued by Quality Service Today, LLC. The Customer must notify Quality
Service Today, LLC within the 1 year warranty period for the warranty to apply. The 1 year
warranty only applies to defects related to the work performed by Quality Service
Today, LLC and does not apply to defects or damage caused by Intervening acts such
as acts of God, weather, nature, intentional or negligent acts by third parties, or
mistreatment or neglect by the Customer.
B. In addition to the above 1 year warranty, Quality Service Today, LLC does hereby
warrant the labor and services provided by it for a period of one year. This one-year
warranty covers service, labor and
parts. This warranty does not apply to defects or damage caused by intervening acts
such as acts of God, weather, nature, intentional or negligent acts by third parties, or
mistreatment or neglect by the Customer. This warranty only applies to claims of
defects in the workmanship by Quality Service Today, I.I.C. The Customer must
promptly notify Quality Service Today, LLC within one year of completion of the work as
evidenced by the issuance of an invoice of the defect to qualify for this warranty.
C. The Customer must allow Quality Service Today, LLC the opportunity to complete
repairs pursuant to these warranties. If the Customer fails to notify Quality Service
Today, LLC of the alleged defects within the applicable time frame and does not allow
Quality Service Today, LLC the opportunity to repair the defect, then the Customer
agrees that it shall have no right or remedy at law against Quality Service Today, LLC
and its owner(s), employee(s), or agent(s) for any claims of defective workmanship or
parts.
D. These limited warranties are the Customer's sole and exclusive remedy against
Quality Service Today, LLC for any alleged defect in completing the work. These
warranties are non-transferrable and belong solely to the Customer. THERE ARE NO
WARRANTIES WHICH EXTEND BEYOND THE DESCRIPTION ON THE FACE
HEREOF UNLESS OTHERWISE PURCHASED. THERE IS NO IMPLIED WARRANTY
OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. Any and ail
parts installed or utilized by Quality Service Today, LLC in performing the work are
covered by the above Syear warranty and the part's manufacturer's warranty ONLY.
The Customer agrees that the Customer shall have no recourse against Quality Service
Today, LLC for any defects in parts utilized by Quality Service Today, LLC in completing
the work beyond enforcement of the 90-day warranty.
2. Limitations of Liability. The Customer understands and agrees that Quality Service
Today, LLC
shall not be liable for any damage caused as a result of existing improper, worn, rusted
or defective plumbing, pipe fixtures, walls, flooring, and/or roofing. The Customer further
understands and agrees that Quality Service Today, LLC shall not be liable for any
incidental or consequential damages or punitive damages which result from materials
supplied or services rendered in completing the work.
3. Warranty. With Service plans
The Services to be performed hereunder are in the nature of plumbing. Service Provider warrants
the Services for three years. in addition, any additional plumbing work or service provided by
Service Provider to Customer while Customer is a party to this Agreement will have the benefit
of all parts and labor warrantied for three years in addition to any additional warranty provided
by the part's manufacturer. If Customer terminates the Services Agreement, then any warranty
will revert to a one-year warranty from the date of completion of the work.
This warranty does not apply to defects or damage caused by intervening acts such as acts of
God, weather, nature, intentional or negligent acts by third parties, or mistreatment or neglect by
the Customer. This warranty does not cover any parts supplied by a customer or third -party and
is strictly limited to parts supplied
A. The Customer agrees that all disputes arising out of or related to the work performed
by Quality Service Today, LLC must be litigated exclusively in the North Carolina
General Courts of Justice in Alamance County. The Customer further waives any
objection to improper venue or lack of personal jurisdiction to any lawsuit instituted in
Alamance County arising out of or related to the work performed by Quality Service
Today, I.I.C.
3. Resolution of Disputes.
A. The Customer agrees that all disputes arising out of or related to the work performed
by Quality Service Today, LLC must be litigated exclusively in the North Carolina
General Courts of Justice in Alamance County. The Customer further waives any
objection to improper venue or lack of personal jurisdiction to any lawsuit instituted in
Alamance County arising out of or related to the work performed by Quality Service
Today, LLC.
B. The Customer further agrees that the Law of the State of North Carolina shall apply
to all disputes arising out of or related to the work performed by Quality Service Today,
LLC.
4. Payment, Attorney's Fees, and Interest.
A. The Customer understands and agrees that payment is due upon completion of the
work and that Quality Service Today, LLC does not accept partial payments or
installments unless agreed to in a signed writing by the Customer and Quality Service
Today, I.I.C.
B. The Customer agrees and acknowledges that this document is evidence of
indebtedness. Accordingly, if Quality Service Today, LLC is a party to any lawsuit
against the Customer, then the Customer agrees and acknowledges that Quality
Service Today, LLC is entitled to recover its reasonable attorneys' fees in the amount of
15%of the total outstanding balance pursuant to N.C. Gen. Stat. § 6-21.2.
C. Pursuant to N.C. Gen. Stat. § 25-3-506, Quality Service Today, LLC will charge a
processing fee of $35.00 for any check returned for insufficient funds, for any
cancellation of a check, or if the payor did not have an account at the bank. In addition,
pursuant to N.C. Gen. Stat. § 6-21.3, Quality Service Today, LLC will seek to recoup,
and the Customer will be liable for, any service charges or other processing fees
charged to Quality Service Today, LLC as a result of any faulty check.

D. The Customer agrees that if all sums due and owing to Quality Service Today, LLC
are not paid upon completion of the work, then Quality Service Today, LLC will charge
interest at the legal rate of 8% per annum until paid. All payments will first be made to
the outstanding interest and only after the Interest is paid off will payments be applied to
the principal.
5. Entire Contract. These terms and conditions and the estimate replace and supersede
any and all previous agreements, written or oral, with respect to the work to be
performed by Quality Service Today, LLC. Should any one or more of the terms and
conditions contained herein be determined to be illegal or unenforceable by a court of
competent jurisdiction, then those terms and conditions shall be severed and all
remaining provisions shall nevertheless remain In effect.
6. Amendments. Amendments to these terms and conditions and to the work to be
performed by Quality Service Today, LLC may only be made in writing signed by Quality
Service Today, LLC and the Customer.
7. Warranty of Ownership. The Customer does hereby warrant and represent that he or
she is the owner or the authorized agent of the owner of the real property which Quality
Service Today, LLC will commence work.
8. Force Majeure. Quality Service Today, LLC shall not be liable under the provisions of
these terms and conditions for damages on account of strikes, lockouts, pandemics,
accidents, fires, delays in manufacturing, delays of carriers, acts of God, governmental
actions, state of war, or any other cause beyond the control of Quality Service Today,
LLC whether or not similar to those enumerated.
9. Mold Release. Quality Service Today, LLC makes no representation or warranty,
express,
implied, or otherwise regarding mold, fungi, rust, corrosion or other bacteria or
organisms. Quality Service Today, LLC shall have no duty or responsibility or liability all
of which is expressly waived by you for losses, fines, penalties, testing, analysis,
monitoring,
10. Septic Systems. The Customer acknowledges and agrees that there are no
guarantees on pumping septic systems or septic inspections as working condition
cannot be determined based on use and Flow.
11. Photograph and Video Release. The Customer grants permission and consent for
Quality Service Today, LLC to take photographs and videos of the work to be performed
and to share those pictures or videos for any legal purpose without further consent from
the Customer.
12. Waiver. Quality Service Today, LLC's failure to exercise or enforce, in whole or in
part, or delay in exercising any right under these terms and conditions, shall not
preclude Quality Service Today, LLC's right to exercise or enforce the same right or any
other right under these terms and conditions.
13. Satisfaction. In the event that the Customer is dissatisfied with the work performed
by Quality Service Today, LLC, the Customer will contact Quality Service Today, LLC
prior to sharing any dissatisfaction to any online forums, websites, social media, or
review sites, which includes but is not limited to reviews to Yelp!, Home Advisor, Angi
Leads, Google, and Facebook (collectively "Websites"). If the Customer contacts
Quality Service Today, LLC to discuss any dissatisfaction, then Quality Service Today,
LLC will take reasonable steps to allow the Customer and the owner of quality Service
Today, LLC to discuss the Customer's dissatisfaction with Quality Service Today, LLC's
work. The Customer agrees that Quality Service Today, LLC will have a reasonable
time to make arrangements to speak with the Customer about the dissatisfaction. If the
Customer violates this provisions and posts a negative review to any Websites prior to
speaking to the owner of Quality Service Today, LLC, then the Customer agrees that
Quality Service Today, LLC will have suffered irreparable harm and consents to
injunctive relief, without a bond, issued by a court of competent jurisdiction to remove
the negative reviews until the owner of Quality Service Today, LLC has an opportunity
to address the Customer's dissatisfaction with Quality Service Today, LLC's work. If the
Customer's dissatisfaction with Quality Service Today, LLC cannot be resolved, then
the Customer agrees to post the full story of any conversations and Interactions with
any negative review shared. If the Customer's comments and/or reviews fail to
accurately and completely capture the conversations and interactions, as determined by
a court of competent jurisdiction, then the Customer consents to the Court awarding
injunctive or other relief requiring the negative review to be removed from any Websites.

I acknowledge satisfactory completion of the work designated on this invoice at 1115 Patterson Road, Durham, NC 27704 USA. I have been given
the opportunity to discuss any issues with this work performed by Seth Clark, and that the premises have been left in a satisfactory condition. I
understand that if my check does not clear, I am liable for the check and any and all charges from the bank. I agree to pay 1.75% per month for
past due amounts (minimum charge $35). In the event that the collection efforts are initiated against me, I shall pay for all associated fees at the
posted rates as well as all attorney's fees and collection costs. I agree that the amount set forth in the space marked "total" is the total price of
$605.661 have agreed to pay today. In the event If I am unsatisfied with the work, I will contact this company prior to posting to any online
forums, websites, social media, or review sites. I agree to give Quality Service Today the opportunity to address my un-satisfaction. Any
comment or review posted anywhere on the Internet must be removed immediately until Quality Service Today is contacted via phone call to
Quality Service Today, and you have spoken directly with the OWNER to address the issue. In the event that my grievances cannot be resolved to

my satisfaction, I agree to post the full story of our conversation and interaction with any social media posting (which includes reviews to Yelp!,
Home Advisor, Angi Leads, Google, Facebook etc.). If my comments and/or reviews are missing the complete story, the post must be
immediately removed and will result in legal action by Quality Service Today.
