Im not sure if these are need but, Attached is the closeout request letter that NCDOT sends to the agencies requesting
that NCDOT close out the site after successful monitoring has occurred. The other letters attached are the confirmation
letters that the agencies send back to NCDOT stating that we can close the sites out and use the mitigation remaining on
the site.

Dave,
The Little Peters Creek site is a DOT-owned mitigation site. It’s not a Division of Mitigation Services (formerly EEP) site,
so there’s no letter with their signature. The attached debit ledger is the documentation DOT provides for DOT-owned
mitigation sites, and has been acceptable to the USACE and DWR in the past.

Creek Mitigation site located in Stokes County, North Carolina. This site is being utilized by the
North Carolina Deparhnent of Transportation as compensatory mitigation for wetland impacts
associated with future projects that have yet to be deteixnined.

Reference your letter dated April 25, 2013, regarding the Little Peter's Creek
Mitigation site located off of NC 704, northwest of Lawsonville, in Stokes County, North
Carolina. This site is being utilized by the North Carolina Department of Transportation
as compensatory mitigation for impacts to jurisdictional waters of the United States
authorized by Department of Army (DA) permit for yet not identified projects.
By copy of this letter, we concur that you have completed referenced DA permit
requirements to monitor the stream mitigation site for channel stability by visual
observation on a biannual basis for iive years or until success criteria were met.
Accordingly, no further monitoring of the site will be required.

Mr. Thomas:
As you are aware, the Department has monitored the Little Peters Creek stream enhancement site
since its construction in June of 2007. The Little Peters Creek site is located in Stokes County
adjacent to NC 704 at approximately'/2 mile east of the intersection of Creasy Road and NC 704.
The site consists of approximately 720 linear feet of upfront stream enhancement mitigation that
upon approval may be used to offset future impacts within the Roanoke river basin. The stream
enhancement was constructed by fencing out the livestock, constructing a floodplain and
installing grade control structures throughout the reach and re-vegetating with appropriate
hardwood species. Per permit requirements, NCDOT was required to monitor the stream
enhancement site for channel stability and survival of planted species by visual observation on an
annual basis for five years or until success criteria were met. ,-
During the monitoring period, annual monitoring reports were submitted to the appropriate
regulatory agencies via the Natural Environment Section's (NES) website:

After five years of vegetation and stream stability monitoring, the Little Peters Creek stream
enhancement site indicates that the planted vegetation is surviving and the streambanks remain
stable at this time. Individual annual monitoring reports are available on NES's website for
2008-2012.

A site visit was conducted on April 24t�', 2013 with the regulatory agencies and NCDOT
personnel present. It was determined at that meeting that the site success criteria had been met
and that no further monitoring would be required. NCDOT requests that the appropriate resource
agencies provide documentation stating that no further monitoring is required and that the site is
closed. Also, please see the attached on-site debit ledger for Little Peters Creek listing the
