On April30, 2023 at 4:30 pm the Town was alerted that the manhole #1067 near the intersection of 108
Plaza Dr Ext. Forest City, NC, 28043 was overflowing. Town personnel worked until 7.30 pm to unstop the
manhole which was clogged with roots and rags. The whole area around 108 Plaza Dr has been jetted and
flushed, videoed and roots removed The Orange County spill estimation guide was used to estimate the spill
volume. Lime was applied to the affected area. The 24hr period notification call was made to Laruen Armeni
and the incident number was given #202300758. The SSO was submitted to the BIMS incident portal.

I have included some before and after pictures of the roots that have been removed from manholes in the
affected area. We will continue to monitor the manhole's to ensure there is no further accumulation.
