I'm just trying to get everything straight on this one with ownership so it's renewed properly. This permit(NCG080455)
is for the facility on the right at 1044 Jimmie Kerr Rd, correct?
We have another permit(Injection well—not a discharge permit, W1400588)for the Flying J Travel Center on the left
(1043 Jimmie Kerr Rd, Flying J—Pilot#682)that is already under Pilot Travel Centers LLC.
The last Ownership change notification we received for NCG080455 was in 2017 (see attached), when it went to PFJ
Southeast, LLC. When did it change to Pilot Travel Centers, LLC? I just want to document it when I make the update,
and make sure the attached owner change in 2017 was for the correct permit. Thank you!

Pilot Travel Centers, LLC is the owner currently and the permit should reflect.
I believe the tanks are under this as well for the site.
We are going to site tomorrow to inspect the OWS and clear a path to the discharge through the woods in anticipation
for the permit reissuance and sampling.

I wanted to verify the Owner name here. We still have PFJ Southeast LLC listed in our system. Is that still the name of
the company? Your letter has Pilot Travel Centers, LLC listed. I know they have several other sites, so that's why I'm
checking.
We have Pilot Travel Centers, LLC listed as an owner entity in our system, but it's not attached to this permit—it's
attached to several other Pilot sites in the State. In the second screen shot is the info for that entity. Since NCG080455

has been inactive for some time, it may have been missed in an owner name change. I can attach it to the correct owner
if that needs to be fixed. Just let me know.
