The revised General Permit was signed on September 20th, 2011 and took effect October 1, 2011. This renewed CoC is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated October 15, 2007 [or as subsequently amended]. A copy of the NCG510000 is enclosed, as well as an updated Technical Bulletin. The Technical Bulletin summarizes the changes from the previous version of NCG510000. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to request an individual permit by submitting an individual permit application. Unless such demand is made, this CoC shall be final and binding. The CoC is not transferable except after notice to the Division. You must notify the Division if you sell or otherwise transfer ownership of the permitted facility. The Division may require modification or reissuance of the CoC. If your facility ceases discharge of wastewater before the expiration date of this permit, contact the Regional Office listed below. Once discharge from your facility has ceased, this CoC may be rescinded. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Water Quality, the Division of Land Resources, Coastal Area Management Act or any other Federal or Local government. Therefore, you are directed to proceed with assessment or remediation activities as required in the Notice of Regulatory Requirements dated November 13, 1997. Effective October 1, 2004, the Department requires that all work following the submittal of the Limited Site Assessment Report (Title 15A NCAC 2L .0405) be preapproved if State Trust Fund reimbursement is anticipated. To comply with this requirement, a completed Preapproval/Claim Authorization Form, encompassing the required remedial activities, must be received in this office within 14 days of the date of this letter. Upon completion of the preapproved activities, you should submit your claim promptly. Reimbursement funds are budgeted based on completed preapprovals, but lengthy delays in reimbursement can occur if claims are not submitted immediately following work completion. Please note that performing assessment and cleanup work that is not required under 15A NCAC 2L .0400 is not reimbursable from the Commercial or Noncommercial Leaking Petroleum Underground Storage Tank Cleanup Funds. (Altamont) on Wednesday, January 9, 2008. As requested, this letter contains a narrative describing the flows since the referenced certificate of coverage was reinstated on August 27, 2007 and the effluent analytical results. Additionally, this letter includes the steps planned to re-establish compliance with the general National Pollutant Discharge Elimination System (NPDES) permit. DESCRIPTION OF FLOW Altamont restarted the system in early September 2007, following an extended period in which the system did not operate (about two years). After restart, flow from the system varied each month as follows: • September 2007 - Approximately 1,000 gallons • October 2007 - Approximately 12,300 gallons • November 2007 — No discharge due to pump failure • December 2007 — Approximately 14,000 gallons • January 2008 - Approximately 3,400 gallons EFFLUENT ANALYTICAL RESULTS Effluent samples were collected in accordance with permit requirements on September 14 and December 4, 2007. The September effluent sample analytical results indicated that all required analytical parameters were below permit limits. With the exception of Methyl -Tertiary -Butyl Ether (MTBE), the P:\OAGregory\D&D Grocery\Correspondence\Ltr-Guerra-Notification.doc Mr. Bob Guerra January 10, 2008 Page 2 of 2 December effluent sample analytical results indicated parameters below the permit limits. The December MTBE sample analytical result was 74.3 micrograms per liter (µg/L). The permit limit is 30 µg/L. In response to the December laboratory analytical result, Altamont turned the system off and notified the North Carolina Department of Environment and Natural Resources (DENR) Division of Water Quality on January 9, 2008. Please find attached the discharge monitoring reports for September through December 2007 (DWQ Forms MR- 1.1) as well as the aforementioned effluent sample laboratory analytical reports and chain of custody forms. STEPS PLANNED TO RE-ESTABLISH COMPLIANCE Altamont plans to replace the activated carbon in the two 1,000 pound liquid phase activated carbon vessels. The groundwater remediation system shall remain off until the activated carbon in these vessels is replaced. Altamont hopes this information is responsive to your needs. The contamination is a result of leaking underground storage tanks (UST). The USTs each contained gasoline. Free phase product has not been witnessed in any of the monitoring wells. The contaminants in the groundwater will be treated so that the effluent wastewater will be below state regulatory levels. The following chemicals were found in the groundwater during the latest sample event (sampled 2/28/98). A copy of the results is included in Appendix A. The treatment system will include a Facet International coalescing oil water separator (Model MCS-11). The separator is capable of handling a maximum flow rate of 25 gpm and a total fluids volume of 46.5 gallons. The detention time for the water is estimated to be 1.3 minutes. However, no free phase product has been detected at the site and is not expected to be encountered in the future. Free product will drain via the product skimmer to a steel holding tank. Product recovered will be pumped out of the holding tank and disposed of off -site. The separator is constructed of carbon steel with internal FRP gel coating. The separator has FRP Slant Rib Silt baffles and polypropylene, impact coalescing material. Catalog cut sheets for the Oil/Water separator are included in Appendix B. Low Profile Air Stripper The treatment system will include a Shallo-vvTray Low profile Air Stripper (Model 2641), which has 4 trays. The blower for the stripper is capable of an air flow rate of 600 cfm and has a EXP motor. The estimated air to water ratio is 179.5 cubic feet per 1 cubic feet. The stripper is designed for an influent flow rate of 25 gpm and effluent water characteristics as follows: Contaminant Effluent Water Concentration Benzene <1 ppb Ethylbenzene <1 ppb • MTBE 1,819 ppb Toluene <1 ppb o-Xylene <1 ppb The stripper will be equipped with a discharge pump and float switch that will control the liquid level in the stripper sump. Technical information for the low profile air stripper is included in Appendix C. Carbon Filtration System The treatment system will include a TetraSoly carbon filtration system (Model HPAF-1000). The system includes two carbon containers in series each containing 1000 pounds of activated carbon. Based on the manufacturer calculations using the Barnaby and Sutcliff computer -modeling program, a breakthrough time of 92 days was estimated. Effluent water from the stripper will be pumped to the carbon system via a discharge pump capable of 30 gpm at 100 feet of head. The carbon system will include a pressure switch and ball valves so that the system can be back -flushed when the drums become clogged. Catalog cut sheets for the carbon filters are included in Appendix D. Recovery Wells 7. What are the well sizes and how many exist? 12, 4" diameter 8. Please state the pump sizes and their location in relation to the treatment used in part 2.E. There are no pumps in the wells. The vapors and water are transported by the use of a vacuum. 9. The remediation system will be equipped with a remote monitoring device that will control the operation of the system and alert appropriate personnel of alarm conditions. The alarm conditions include a high level float in the stripper sump, a high level float in the oil/water separator, and a high level float in the product storage tank. Gregory, Inc., Altamont Environmental, Inc. (Altamont) is applying for renewal of the National Pollutant Discharge Elimination System (NPDES) permit for the D&D Grocery site. The following letter addresses the reasons for NPDES permit renewal and the application requirements. BACKGROUND On March 23, 2001, a groundwater and vadose zone vapor (dual phase) extraction and treatment system began operating at the D&D Grocery site to treat a release from gasoline underground storage tanks (Figure 1 and 2). The treated groundwater (effluent) was discharged to Ivy Creek under NPDES permit NCG510412. The groundwater remediation system was turned off on December 14, 2004 as instructed by the North Carolina DENR Underground Storage Tank (UST) Section. No free product was documented in groundwater wells during the last two sampling events prior to the shutdown of the remediation system in 2004. However, free product was present in one monitoring well (MW-2) during the most recent sampling event in March 2007. In a letter dated December 27, 2006, the DENR required remediation activities to resume. Remediation activities include operation of the groundwater remediation system and discharge of the treated effluent from the system. Therefore, Altamont is applying for renewal of the NPDES permit. TREATMENT COMPONENTS The remediation system includes twelve extraction wells and a skid mounted treatment system. Vapor is extracted from the wells using an oil -sealed liquid ring pump and discharged to the atmosphere following treatment with activated carbon. The recovered groundwater is also extracted by the liquid ring pump and treated by an oil/water separator, an air stripper, and two activated carbon vessels. The calculated breakthrough time for the activated carbon units is included as Appendix B. ANALYTICAL MONITORING DATA During operation of the system, the measured effluent compound concentrations were below limits established by the permit. Analytical monitoring data for the final sampling event are included as Appendix C. DISCHARGE ALTERNATIVES According to NPDES permit guidance, an evaluation of alternatives to discharge is required for new or expanding facilities. This application is for a permit renewal, therefore an Engineering Alternatives Analysis is not required. However, the alternatives to direct discharge at the site are summarized below. It is not feasible to discharge the effluent to a Municipal or Regional Sewer Collection System because the closest sewer service provided by the Metropolitan Sewerage District of Buncombe County (MSD) is approximately 5.5 miles away from the site. A map indicating the site location and the MSD boundary is included as Appendix D. Subsurface disposal and spray irrigation are not feasible because the land is not owned by the responsible party. In addition, drinking water wells are located within 100 feet. As part of the renewal application, the completed Notice of Intent and all supporting documents are included.
