REVISION 10/30/2023: This report has been revised to include previously omitted flow data.
Frequency Violations for Turbidity and Flow: During this period, turbidity and flow measurements were observed but not recorded, resulting in incomplete monitoring reports. To prevent such occurrences,
operators are now required to document measurements in both Envirolink's Computerized Maintenance Management System (CMMS) and the field bench sheets. Moreover, all staff members have undergone
additional compliance training, are receiving additional training and support on a once weekly basis, and are mandated to undergo refresher training annually.
(1) frequency violation for pH: A singular pH frequency violation occurred on 4/25, where the measurement was observed but inadvertently left unreported. With this exception, the facility has otherwise been
compliant in meeting all pH monitoring requirements.
Missed monthly sampling: The facility is required to conduct monthly monitoring for BOD, Fecal Coliform, TSS, Total Nitrogen, Ammonia Nitrogen, Nitrate, TKN, and Phosphorous. The misstep occurred when
the recently onboarded operator inadvertently labeled the monthly samples as "monitoring well" samples on the chain -of -custody form, rather than specifying that they should be analyzed as effluent wastewater
samples as stipulated by the permit. Consequently, the samples were improperly analyzed, rendering them invalid. This oversight unfortunately resulted in the subsequent exclusion of the required monthly
samples, resulting in a monitoring violation.
