Please find this as the response to the comments as shown on the letter from you, dated July 31, 2015. All comments
were addressed as follows:
1. Please provide a new Page 4 of the permit application (PCN) with a modified Table C.3 that lists each impact
area separately and accounts for all temporary as well as permanent impacts and any modifications made to the
project in response to this letter. Proposed impacts include culvert impacts, streambed material excavation, and
channel relocation impacts.
Page four of the PCN has been modified. The stream impacts have been broken down into removal of existing
culvert, installation of new culvert, stream excavation, excavating new stream channel and tilling existing
channel for relocation. Crossing 1 and Crossing 2 includes the impact of replacing the culvert and excavation
of accumulated sediment. Crossing 5 includes the Installation of the new culvert (45') and filling the existing
stream (52') for a total impact of 97'. Crossing 7 inclui;les the remoyal of the existing culvert (2-1') and filling
the•existing stream (70') for a'total of 91'. The new culvert was not considered an impact as it was not installed
within the existing stream channel.
Please provide a typical cross - section detail for each area of proposed streambed excavation and stream
relocations. Each cross- section shall include the proposed bankfull width, depth, area, streambed slope, and
stabilization method (e.g. coir matting detail). The proposed cross - section dimensions shall reference the
appropriate regional curve data or reference reach data.
Cross: section details have been added to sheet SD -8.1 for CR -2, CR =S and CR -7. Data was obtained through
the use of regional curve data and regional curve equations.
3. Please provide the proposed thalweg profile for the stream relocations at CR 2 and CR 5.
Proposed thalireg profiles have been added as sheet SD -6.1 f'o`r both CR2 and CRS.

Other changes to the plans include:
1. Incorrect sheet numbers have been revised.
2. After discussions with pipe suppliers, the wingwalls have been modified.

The Division of Water Resources (DWR) has performed a review of a 401 Water Quality Certification
application received on June 25, 2015 for the above referenced project. The Division has determined that your
application is incomplete and cannot be processed. The application is on -hold until all of the following
information is received:
1. Please provide a new Page 4 of the permit application (PCN) with a modified Table C.3 that lists
each impact area separately and accounts for all temporary as well as permanent impacts and any
modifications made to the project in response to this letter. Proposed impacts include culvert
impacts, streambed material excavation, and channel relocation impacts. [15A NCAC 02H
.0502(b)(2) and (c)(2)]
2. Please provide a typical cross - section detail for each area of proposed streambed excavation and
stream relocations. Each cross - section shall include the proposed bankfuli width, depth, area,
streambank slope, and stabilization method (e.g. coir matting detail). The proposed cross - section
dimensions shall reference the appropriate regional curve data or reference reach data. [15A

3. Please provide the proposed thalweg profile for the stream relocations at CR 2 and CR 5. [15A

If all of the requested information is not received in writing by the response due date, the DWR will be unable
to approve the application and it will be returned. The return of this project will necessitate reapplication to the
DWR for approval, including a complete application package and the appropriate fee.
