I have addressed your comments in the November 9, 2009 in the following manner:
1). The application has been submitted with an original signature.
2.) The OM form is resubmitted but does not have a Notary, per the Base's discussions
with you and your department concerning this issue. It is my understanding that the
Notary requirement has been waved
3.) The typo on the elevation has been corrected to reflect the 301.40 from the plan
sheet.
4.) The calculation in question was an example calculation that was not updated with
the pertinent information for this basin. The actual calculation is based on links to
cells, so it was calculating correctly. The example calculation in question has been
updated to reflect the values relevant to this calculation.
