Subject: Request for Additional Information
Stormwater Project No. SW6080804

The Division of Water Quality Central Office received a Stormwater Management Permit Application for
the subject project on August 25, 2008. A preliminary review of that information has determined that the
application is not complete. The following information is needed to continue the stormwater review:
The person that signed the application information is not listed on the Secretary of State's
Corporation database. The person signing must be at least a vice-president in the corporation.
Otherwise, please provide documentation of his signatory authority for the Corporation.
2. Please provide supporting calculations to show that the velocity in the swales to the
bioretention cell is non -erosive (I fps for mulch cells, 3 fps for grass cells).
3. The media depth of 2 ft provided in the design is for sod grass cells only. The planting plan
shows grass and other plants. The media depth will need to be increased to 3 ft if it will be
planted with a combination of grass and plant/mulch areas.
4. A minimum 4 inch layer of clean sand must be provided between the media and the washed
stone layers per Figure 12-4 in the BMP Manual. Also, the cross-section of the cell should
show the separation to the SHWT.
5. The geolechnical report submitted does not contain the required information needed for our
review. A soil boring, within the footprint of the proposed burp, is needed. The soil boring
report must contain an estimated seasonal high water table (SHWT) elevation. We require that
the SHWT be at least 2 feel below the bottom of the bioretention cell.
6. Please delineate any wetlands on the site; or add a note to the plans that states there are none.
7. Please show on the plans how the roof area will not drain to the proposed cell (ex. show roof
leader locations). If any of the imperviousareasbeing considered as part of the "existing`
BUA (roof area) cannot be directed away from the cell, they must be accounted for in theN'°o".etnearohna

The information provided in the table of part 111.9 of form SWU-101 should be filled in as one
drainage area per burp. For this specific project, please complete one DA for the bioretention
cell, and another DA for the area not treated.
9. In order to prevent clogging, the construction sequence should require that the biorctention cell
not be installed until its drainage area is stabilized.
10. Volume in excess of the 1" treatment volume must bypass the bioretention cell. This bypassed
volume must be evenly distributed across a minimum 30' long vegetated filter strip. The filter
strip must be designed in accordance with Chapter 13 of the BMP Manual.
11. Please show a proposed drainage easement connected to a public ROW around the burp to
provide for access for maintenance and inspections.
Please note that this request for additional information is in response to a preliminary review. The
requested information should be received by this Office prior to October 3, 2008, or the application will
be returned as incomplete. The return of a project will necessitate resubmittal of all required items,
including the application fee.
If you need additional time to submit the information, please mail or fax your request for a time extension
to the Division at the address and fax number at the bottom of this letter. The request must indicate the
date by which you expect to submit the required information. The Division is allowed 90 days from the
receipt of a completed application to issue the permit.
Please reference the State assigned project number on all correspondence. Any original documents that
need to be revised have been sent to the engineer or agent. All original documents must be returned or
new originals must be provided. Copies are not acceptable. If you have any questions concerning this
