received on March 3, 2023. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on June 26,
2023.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through B, or failure to provide the additional information on or before the above -requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A. Lease Agreement:
1. The Emergency Application Area for Permit WQ0002004 is owned by Mr. Kenneth Edwards, not
Bass Farms, Inc. (the permittee). Pursuant to 15A NCAC 02T .0504(f)(3), please submit an
agreement to lease the property that is written, notarized, and signed by both parties, indicating the
intended use of the property, accompanied by a plat or survey map. Lease agreements shall adhere
to the requirements of 15A NCAC 02L .0107.
B. Compliance Boundary (15A NCAC 02L):
In accordance with the 15A NCAC 02L Groundwater Classifications and Standards rules (hereby
referred to as the 02L Rules), a compliance boundary is defined as a boundary around the waste
disposal area of a waste disposal system at and beyond which groundwater standards may not be
exceeded. The Emergency Application Area for Permit WQ0002004 is owned by Mr. Kenneth
Edwards, not Bass Farms, Inc. (the permittee). Since the compliance boundary for the Emergency
Application area is located on land not owned by the permittee, Bass Farms, Inc., with Mr. Edwards,
must execute and file in the Nash County Register of Deeds an easement running with the land that
contains a notice of the permit (including the permit number, a description of the type of permit,
and the name, address, and telephone number of the permitting agency) pursuant to 15A NCAC
02L .0107(k).
