Dear Mr. Key:

Division of Water Resources staff Kristen Potwora conducted a review of the 2022
Residuals Annual Report. This review reflected non-compliance with the subject permit.
The following deficiency was noted:

1. The 2022 Residuals Annual did not contain a Class A Annual Distribution &
Marketing Certification and Summary Form (DMSDF). This was also noted in
correspondence dated April 5, 2021, and April 13, 2022. Please submit a
properly completed DMSDF form in all future annual reports for the subject
Class A permit. If necessary, please also include supplemental forms (DMSDF-
Supp) to provide information that will not fit on form DMSDF.

Please refer to the enclosed inspection report for additional comments and observations.

On August 3, 2023, Division of Water Resources staff Kristen Potwora completed a review of the 2022 Residuals Annual
Report for WQ0038071. This review reflected non-compliance with the subject permit. The following item of deficiency was
noted:
1.The 2022 Residuals Annual did not contain a Class A Annual Distribution & Marketing Certification and Summary Form

Other Observations –
•No land application events occurred in the 2022 calendar year. Application events are planned for the Town of Forest
