IV. ADDITIONAL REGIONAL STAFF REVIEW COMMENTS/ATTACHMENTS (Optional/If Needed)
Duringthe hpection, we examined the 10 wells identified on the map included with the permit, as well as the
treatment system building (Fi..u�). In addition, two unlabeled wells were observed duringthe hpection.
The twelve wells observed were secure, locked, and had identification (ID) plates attached (Photo Lofz),
although a number of the ID plates were unlabeled, illegible, or included the incorrect well number. Wells
northwest of US Highway 1 were overgrown, and difficult to find.
