Subject: Request for a NW 3 perinit for the replacement of Bridge # 36 over Smith Creek
on SR 1143 (Sladesville-Credle Rd), Hyde County. WBS # 17BP.1.R.63
Dear Ms. Wheeler,
The North Carolina Department of Transportation is requesting the issuance of a
NW 3& 12 permit, along with the corresponding 401 certification for• the replacement of
Bridge # 36 over Smith Creek on SR 1143, in Hyde County. The bridge is located about
0.13 rniles south of the intersection with SR 1152. Smith Creek is within the Tar-Pamlico
River Basin, and drains to the Pungo River, 1.5 miles to the west. Smith Creek is listed as
an ACOE Section 10 waters and classified by DWQ, as class "SC, NSW" waters. The
N.C. Marine Fisheries and the NCWRC reviewed this site and determined an in-water
woz•k moratorium will not be required at this location.
Bridge # 36 was built in 1959, and consists of 3-spans with a wood deck (asphalt
wearing surface), supported by wood joist and caps on timber piles. The existing structure
is 52 ft. long by 20 ft. wide. This structure will be entirely removed using NCDOT's
BMP's and guidelines for bridge removal. The pr•oposed bridge will be replaced along its
existing alignment. The new bridge will be a 2-span structure totaling, 90' in length by
28' wide. It will be comprised of a precast concrete deck and substructure. Jurisdictional
wetland and surface water iinpacts have been avoided and minimized to the maximum
extent practicable. The resulting permanent impacts associated with roadway fill are
0.034 acres (1481 sq. ft.) of road shoulder coastal fi�inge wetlands. There will be 0.004
acres (200 sq. ft.) of permanent hand clearing, established for long term maintenance
around the bridge end bents. This will allow for revegetatioil of coastal wetland species,
but will be periodically mowed by maintenance forces. There is 0.032 acres (1419 sq. ft.)
of proposed temporary hand clearing, which will provide a clear work/maintenance area
for erosion control devices. This area will be allowed to revert back to fully vegetated
status, once the project is completed. There is a 0.002 acres (121 sq. ft.) of permanent
surface water impact associated with rip-rap stabilization around the end bents. The

wetland impact areas are located and detailed on the provided permit drawings. Coastal
wetland impacts have been initigated for through the Division of Mitigation Service. A
copy of their acceptance lettel� is provided with this application and documents they will
provide 0.038 acr•es of Coastal Marsh mitigation.
There will be no deck drains located on the bridge. Storm water has been routed to
an outlet located on the southwest quadrant of the bridge. This outlet has been placed at a
feasible location that would allow for vegetated buffer conveyance, prior to entering
surface waters.
Traffic will be maintained offsite by the use of a detour route. There will be no
permanent utility relocations on this project. A temporary relocation of a power line and
support pole will be required during project construction. This temporary relocation will
not impact any jurisdictional wetlands or surface waters.

coastal mazsh mitigation for the subject project. Based on the information received from you on July 20, 2015, the
impacts are located in CU 03020104 of the Tar-Pamlico River basin in the Northern Outer Coastal Plain (NOCP)
