Dear Mr. Stuetzle:

It has been brought to our attention that thirty-nine (39) temporary groundwater monitoring wells were
installed at the subject site without first obtaining a monitoring well construction permit from this office.
15A NCAC 2C .0105(a)(3) states that no person shall locate or construct any temporary or permanent
monitoring well or monitoring well system, including wells installed using direct -push technology (DPT)
or Geoprobe® technology, designed to penetrate an aquifer to obtain groundwater data on property not
owned by the well owner until a permit has been issued by the North Carolina Department of Environmental
Quality.

Please ensure that in future assessments and corrective actions at this site, all required well permits are
obtained before the wells are constructed. If this deficiency occurs again, a Notice of Violation may be
issued.
