Dear Mr. Wiseman,
In response to your permit major modification request received on June 16, 2023, and subsequent
additional information received on August 7, 2023, we are forwarding herewith a modified Attachment B
certified August 11, 2023, to the City of Asheboro for the subject program.
Modifications to the subject Attachment are as follows:
➢ The Division has added Fields NC-GU-028-01, NC-GU-028-02A, NC-GU-028-02B, NC-GU-
028-03, NC-GU-028-04, NC-GU-028-05, NC-GU-028-06, NC-RD-042-04, and NC-RD-042-
05, resulting in an increase of 100.47 acres.
➢ The Division has increased the acreage of Field NC-RD-042-02 from 8.60 acres to 14.79 acres.
The modified Attachment B is effective from the date of certification until September 30, 2025,
shall replace Attachment B of Permit No. WQ0001684 issued on October 10, 2019, that was previously
certified on September 14, 2022, and shall be subject to the conditions and limitations therein, as well as
those specified in the above -referenced permit. Please replace Attachment B of Permit No. WQ0001684
certified on September 14, 2022, with the modified Attachment B. The Permittee shall submit a renewal
application no later than April 3, 2025.
This certification can be contested as provided in Chapter 150B of the North Carolina General
Statutes by filing a Petition for a Contested Case Hearing (Petition) with the North Carolina Office of
Administrative Hearings (OAH) within 30 calendar days. Requirements for filing a Petition are set forth
in Chapter 150B of the North Carolina General Statutes and Title 26 of the North Carolina Administrative
Code. Those interested in filing may access additional information regarding the requirements for filing a
Petition and Petition forms at the OAH website or by calling the OAH Clerk's Office at (919) 431-3000.
A party filing a Petition shall serve a copy of the Petition on the Department of Environmental Quality's
Office of General Counsel at 1601 Mail Service Center, Raleigh, NC 27699-1601. If the party filing the
Petition is not the Permittee, then the party shall also serve the Permittee pursuant to G.S. 15013-23(a).

1. The Division has approved a re -defined compliance boundary for this land application site. This re -defined compliance boundary is located 50 feet from the water supply well on the property. The
Permittee shall still meet the 100 foot setback between wells and the land application area.
2. This land application site is currently unsuitable for residuals application. The Permittee shall not land apply residuals until the Field Owner clears the site and makes it suitable for residuals
application, the Winston-Salem Regional Office performs a detailed site review is performed, and the Winston-Salem Regional Office gives written permission for the Permittee to land apply
residuals.
Pursuant to a setback waiver recorded in the Randolph County Register of Deeds, reduced setbacks from the Class B land application area boundary to any habitable residence or place of assembly
under separate ownership are as follows:
