Dave,
As a result of 2+ inches of rainfall on Sunday night, July 23, sediment losses occurred at two additional locations for this
same project. On Tuesday, July 25, DOT Division 10 environmental staff, Thomas Smith — DOT Roadside Environmental
Unit and contractor representatives conducted onsite inspections for all known sediment loss locations. Cleanup crews
are currently working at all three sites to clean up the sediment losses and re-establish sediment and erosion control
measures. Before / after pictures and bucket counts will be submitted to you once cleanup activities are concluded.

Dave,
Sediment losses have occurred at 26+50-Y5RPB, plan sheet 59. Sediment entered a small wetland (<0.01 acres). The
site inspector missed seeing this sediment loss earlier. Clean up crews are currently cleaning out the impacted
wetland. I will send pictures and buckets totals as soon as they finish their work. Once sediment is removed then a
riparian seed mix will be applied to the wetland.
