North Carolina NCDEQ Incident#48083 HES Project#91041 Dear Mr. Snider: On behalf of our client (Enterprise Holdings, LLC - EHLLC), Highlands Environmental Solutions, Inc. (HES) is submitting this application to install one groundwater monitoring well at the subject site. The location of the site is depicted on Figure 1 and 2, included in Attachment A. The monitoring well is being installed as a condition of the completion of a Phase I Limited Site Assessment (LSA) per the Notice of Regulatory Requirement (NORR) letter dated July 6, 2023. A copy of the letter is included in Attachment B. The LSA is being required following the closure of one underground storage tank at the site. The property is owned by the PTI Airport Authority. As such, a Monitoring Well Permit is required. The scope of work for the LSA includes the installation of one groundwater monitoring well in or immediately adjacent to the former UST basin. The well will be an EPA Type II groundwater monitoring well. The well will be installed to a maximum anticipated depth of 30 feet below grade, constructed of two-inch Schedule 40 PVC materials, and completed with a flush mount metal cover and locking expansion cap. A schematic of the proposed well is included as Figure 3. The completed Monitoring Well Permit Application is included in Attachment C. Records indicate that you are the owner or operator of this UST system. Therefore, as a responsible party, you must comply with the initial response and abatement action requirements of the Title 15A of the North Carolina Administrative Code(NCAC), Subchapter 02L .0404 and,if applicable,the assessment and reporting requirements of Title 15A NCAC 02L .0405,within the timeframes specified in the attached rules.
