4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: *
Concrete channel within industrial/commercial area. Concrete has failed on one side of the channel and slabs are dislocated such that runoff is eroding soils beneath the slabs. The other
side and the channel bottom also have areas with cracks.

Purpose of the project is to repair failed areas of concrete channel.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:
Project will involve demolition of failed concrete slabs on left bank (left based on looking in downstream direction) and installing and keying -in new slabs at bottom and top of bank. Prior to
pouring new slabs, unsuitable soils behind existing concrete will be removed and replaced with suitable and adequately compacted fill. Right bank repairs will be relatively minor and
limited to repairing cracks in existing slabs. Flow in stream is to be maintained by installing concrete median barrier covered with impervious material such as polypropylene or a geotextile
and only working on one side on the stream at a time.

6i. Comments:
The temporary buffer impacts on the North side of the stream are as follows: a gravel construction entrance in Zone 1 and Zone 2, a staging and
laydown area in Zone 1, a geotextile dewatering dirt bag in zone 1, and a silt fence in Zone 1. All of these temporary impacts are for erosion and
sediment control and will be removed after the project is complete.
The temporary buffer impacts on the South side of the stream are as follows: a staging and laydown area in Zone 1 and Zone 2, a drivable area for
construction vehicles in Zone 1 and Zone 2, and a silt fence in Zone 1. All of these temporary impacts are for erosion and sediment control and will be
removed after the project is complete.
The Jordan Lake Water Supply Watershed Buffer Rules Table of Uses states that streambank or shoreline stabilization is allowable in buffer zones. Final
grading for the project will return the area to the buffer topography prior to erosion and failure of the stream bank, with the intent of re-establishing diffuse
flow through the buffer zone. All areas will be seeded and monitored until turf is adequately established.

Alternatives to repair using ecologically preferable materials were thwarted by site limitations and the FEMA for no increases to Base Flood Elevations in
an area where structures are at risk of flooding in a 1 % annual chance storm. All impacts are temporary.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Work will be staged such that low flow in channel can be maintained throughout the project without requirement for a pump -around.
