Muddy Creek Pump Station Replacement
Temporary Domestic Wastewater Pump & Haul Permit
Stokes County
Dear Mr. Barrow,
Per Permit Condition 11.1 an accurate record of all pump and haul activities must be
maintained and these records shall be submitted monthly to the Supervisor of the Water
Quality Regional Operations of the Winston Salem Regional Office. Based on this
permit condition, the following deficiency was noted:
A. A review of available records show that no monthly reports have been submitted
since the issuance of this permit. Within ten (10) days of receipt of this notice,
please submit the monthly reports for the months of April, May, and June.
Moving forward, please submit monthly reports on the Wednesday
following the end of the month.
Our office appreciates your attention to the above listed deficiencies. If you or your staff
