Cover Letter: 1. No Comment B. Application Fee: 1. No Comment C. Application (SFRWWIS 06-16): 1. The submitted Form SFRWWIS 06-16 states that Jamel Mathis is the applicant for this permit. Everywhere else in the package lists Brandon R. Combs and Jacquelyn C. Combs as the applicants. Please clarify which person(s) is the correct applicant. 2. Within Section VII. of the application, the soil series within the Soil Evaluation Recommendations is listed as Siloam. The Irrigation System Information, however, lists the Dominant Soils Series as Lynchburg/Rains Modified with Loamy Topsoil. Please clarify which soil series is correct. D. Existing Permit: 1. Not Applicable E. Property Ownership Documentation: Per 15A NCAC 02T .0606(c) all required setback waivers shall be provided. The second setback waiver provided (for 8061 Shallowford Ridge Road) is for the proximity to property lines. The intent of this waiver seems to be to allow irrigation up to 40 feet from the Permittee's residence. If this is the case, then the waiver should have been for "Places of Assembly" and not "Property Lines". Additionally, the current setback for habitable residences owned by the permittee and utilizing spray irrigation is 200 feet and not 100 feet. F. Soil Evaluation: 1. A large portion of the application area is currently covered with trees. Achieving adequate coverage in this area through a spray irrigation system may necessitate the removal of some or all of these trees. There is a possibility that the removal process could alter both underlying soil conditions and the overall topography of the site. What is the current plan to maintain the soil characteristics outlined by the soil scientist? Mr. and Mrs. Combs April 10, 2023 Page 3 of 5 G. Engineering Plans: Per 15A NCAC 02T .0604(c)(1), engineering design documents for the entire system shall be provided. Please respond to the following: 1. It looks as though an audible/visual high water alarm exists within the recirculation tank, however, it has not been labeled. Please label this component in the plans. 2. The profile view provided shows the Recirculation Tank feeding into the Field Dosing Tank through a bottom connection with no obvious path through the UV disinfection unit. Please explain how this effluent is being disinfected. 3. The Soil Amendment Notes on sheet 3 of the Engineering Plans seems to call out 12 inches of fill material to be applied over the entire site. Please clarify whether this fill material will only be applied in the designated fill areas. H. Specifications: 1. No Comment I. Engineering Calculations: 1. During review of the provided engineering calculations, no justification for the field size was given. The Soils Report states that 17,202 square feet (0.395 acres) of land is fit for irrigation with a maximum irrigation rate of 24.01 inches per year. The design currently lists the area of application to be 0.424 acres with a design rate of 15.23 inches per year. While the design rate is within the maximum, the application area exceeds the maximum irrigable area. If the current design rate were to be applied evenly over the maximum field size of 0.395 acres, the capacity of the system would only be 447.5 gallons per day instead of the required 480 gallons per day for a four bedroom house. Please ensure that the field size and loading rate fit within the parameters provided by the Soil Scientist and update the submitted Calculations to reflect rationale used for field sizing pursuant as required by 15A NCAC 02T .0604(c)(3). Mr. and Mrs. Combs April 10, 2023 Page 4 of 5 J. Site May: 1. Following the most recent site visit by regional staff, stormwater/drainage ditches and an intermittent stream were observed. These features have not been buffered out. Regional staff have mapped these locations in the following site map: Soil mapping units are also not observable on the engineering site map. Additionally, setbacks only seem to be recorded for structures within 400 feet of the application boundary. 15A NCAC 02T .0604(d)(2) state that all locations within 500 feet of the waste treatment, storage, or disposal site, including a delineation of review and compliance boundaries shall be illustrated. Please illustrate all buffers and compliance/review boundaries pursuant to 15A NCAC 02T .0604(d) and provide soil mapping units on the contour map. K. Operation and Maintenance Plan: The provided Operation and Maintenance Plan consists of manufacturers design details for pertinent tanks and the Innovative Wastewater System Approval for the E-Z Treat System. Per 15A NCAC 02T .0604(f), the Operation and Maintenance Plan shall address: "...routine inspections, maintenance schedules, troubleshooting, and a layman's explanation about the wastewater treatment and irrigation disposal systems shall be submitted to the Division by the applicant." Please provide an Operation and Maintenance Plan that meets this definition. Mr. and Mrs. Combs April 10, 2023 Page 5 of 5 L. Operation and Maintenance Agreement: 1. Per 15A NCAC 02T .0604(h), an Operation and Maintenance Agreement shall be submitted for the facility. The agreement provided is listed for 8069 Shallowford Ridge Road, Lewisville NC 27023. The permit application, however, is for 8061 Shallowford Ridge Road, Lewisville NC 27023. Please provide a corrected Operation and Maintenance Agreement. M. County Health Department Denial Letter: 1.
