A review has been conducted of the self -reported Sanitary Sewer Overflow (SSO) 5-Day Report submitted by Old
North State Water Company LLC. This review has shown the Old North State Water Company LLC to be in
violation of the requirements found in the subject permit and/or North Carolina G.S. 143-215.1(a)(1). The
violations that occurred are summarized below:

duration, gravity, impacts to public health, fish kills and other factors.
