Director Jared Anderson 2049 Bethlehem Church Road Reidsville, North Carolina 27320 NORTH CAROLINA Environmental Quality May 30, 2023 SUBJECT: Compliance Evaluation Inspection NPDES General Wastewater Discharge Permit - NCG550000 Certificate of Coverage: NCG550365 Single Family Residence 2049 Bethlehem Church Road, Reidsville Rockingham County, North Carolina Dear Mr. Anderson: On May 12, 2023, Division of Water Resources staff Jim Gonsiewski conducted a routine compliance inspection of this permitted facility. As owner, you were present for the inspection. A review of records, treatment components, and system operations was completed. Findings from the inspection are summarized in the enclosed report. This inspection reflects non-compliance with the subject permit. Observations made during the inspection indicated that the system appeared to be operating properly. Mr. Gonsiewski provided you with a list of the proper chloring tablet type and commercial sources of the tablets. Please ensure that the septic tank is pumped out every three to five years. Also, if you have not done so already, please fill out the enclosed Permit Name/Ownership Change Form and submit it to the address noted on the second page of the form. The following areas of noncompliance were noted: 1. You are required to have the discharged water sampled and analyzed by a certified laboratory annually. Records of monitoring should be maintained for at least 3 years. At any time, the Division may request to review documentation required to be kept by the Permit to determine compliance with its conditions. Attached please find a list with the names and phone numbers of two laboratories. 2. Additionally, you are using an incorrect type of chlorine tablet. The permit requires that wastewater grade tablets (calcium hypochlorite) shall be added as needed to provide proper chlorination (pool chlorine tablets shall not be used). As mentioned above, you were provided a list of the proper chlorine type and commercial sources of the chlorine during the inspection. Your immediate attention in addressing the above noncompliance issues is appreciated. Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) On May 12, 2023, Division of Water Resources staff Jim Gonsiewski conducted a routine compliance inspection of this permitted facility. Jared Anderson, property owner, was present during the inspection. A review of records, treatment components, and system operations was completed. This inspection reflected non-compliance with the subject permit. The septic tank is located on the south side of the house. Waste flows from the residence to the septic tank, to the sand filter, and then to the chlorinator tubes. The effluent then discharges to an unnamed tributary east of the residence. Areas of noncompliance • The permit requires that the effluent is sampled and analyzed by a certified laboratory annually. Records of monitoring should be maintained for at least 3 years. At any time, the Division may request to review documentation required to be kept by the Permit to determine compliance with its conditions. • The wrong type of chlorine tablet is being used. The permit requires that wastewater grade tablets (calcium hypochlorite) shall be added as needed to provide proper chlorination (pool chlorine tablets shall not be used). Staff provided Mr. Anderson with a list of the proper chlorine type and commercial sources of the chlorine during the inspection. Observations: • Mr. Anderson stated that he has the septic tank pumped every two years. He stated that the last time the tank was pumped was 2022. The permit requires that the septic tank be pumped out at least once every 5 years or when the solids level is found the be more than 1/3 of the liquid depth in any compartment, whichever is greater. • No evidence of leakage or odors was noted around the system. • Mr. Anderson stated that incoming water is provided to the residence by Dan River Water, Inc. The residence was constructed in 1977. Mr. Anderson purchased the property from his mother in 2007. Currently, there are four people living in the home. • The sand filter was replaced in 2007. The entire system was replaced in 2010. • Mr. Anderson needs to submit a Change in Ownership form. This was provided to him at the time of the inspection.
