3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
The project site currently consists of an active playground area. The general land use in the vicinity of the project
consists of the Smithfield Community Park and residential land use.

The purpose of the proposed project is to renovate the existing play area to construct the proposed inclusion park,
which will provide a recreational area accessible to children of all abilities, including physically challenged and
special needs, which will promote interaction.
3f. Describe the overall project in detail, including the type of equipment to be used:
Inclusion Park is a playground to serve children of all abilities. Some of the features within the park will include
ADA accessible walkways (concrete), (2) Picnic shelters, natural play elements (i.e. a hill slide, natural boulders
for climbing, and a sand "stream "), commercial play equipment, playground surfacing (both poured -in place and
engineered wood fiber), and an open lawn area for the community to hold events. Nature is a large focus of
Inclusion park with a large emphasis on using a wide variety of plant materials to create outdoor "rooms" that
separate spaces for kids to play.

W &R conducted a site review and determined that there are no wetlands within the project area. The only
jurisdictional feature is the channelized stream which runs along the eastern edge of the proposed Inclusion Park.

D. Impact Justification and Mitigation
1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project.
The proposed Inclusion Park is being constructed within the location of an existing play area, and the buffers
currently consist of mowed grass. The project has been designed to keep all playground structures out of the
buffer. Additionally, the proposed changes to the playground surfacing will occur only in areas where there is
existing mulch, which constitutes and existing and ongoing use.
1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques.
The park has been designed to keep all structures out of the buffers except for the proposed walking trails, which
are allowable.

E. Diffuse Flow Plan
All buffer impacts and high ground impacts require diffuse flow or other form of ® Diffuse flow
stormwater treatment. Include a plan that fully documents how diffuse flow will be ❑ Other BMP
maintained. If a Level Spreader is proposed, attach a Level Spreader Supplement Form.
If due to site constraints, a BMP other than a level spreader is proposed, please provide a
plan for stormwater treatment as outlined in Chapter 8 of the NC Stormwater BMP
Manual and attach a BMP Supplement Form.
The proposed walking trails within the buffers will be constructed at grade, which will
allow runoff to continue to sheetflow across the buffers, maintaining the existing diffuse
flow. Additionally, the proposed Inclusion Park will not generate enough stormwater or
nutrient discharge to require stormwater management, and as such, there won't be any
discharges requiring BMPs such as level spreaders.
