January 4" ,
I still have not seen the updated page of the renewal application. Please remember that the due date for this additional
information request is due by Monday, January 30`h, 2023. If the additional information cannot be provided by this date,
I unfortunately will have to return the application in accordance with 15A NCAC 02H.1045(4)(a)(ii) to allow for any
additional time needed. If you wish to pursue the renewal after the application is returned, you must reapply to the
Division and submit another processing fee.
As stated before, the minor nature of the additional information request allows the option for a scanned copy of the
page one of the application to be sent via email. I can print the corrected page here at the office.

I have reviewed the information that was dropped off to our office on December 14, 2022, in response to the additional
information request that was sent on December 9, 2022. A review of the items submitted indicate that after an
additional review, the items requested have not yet been met. I will attempt to explain what the items that were
requested on December 9, 2022, and what is needed in order to proceed with your stormwater permit renewal.
Item #1 is attempting to get a complete response on Section A.4 of the page one renewal application that was submitted
to our office on December 5, 2022. Section A.4 was left blank. This question is asking what changes to the project were
made. If no changes occurred, please complete Section A.4 with none or NA.
Item #2 is requesting the mailing address on the application match what is listed on the NC Secretary of State business
registration. The mailing address listed on the NC SOS webpage is 2709 Leader Circle. Currently on the application under
review, a P.O. Box is listed. For permitting purposes, I must have the address that is listed on the NC SOS registration. If
you would like for the renewed permit to also be mailed to the PO Box, I can certainly be sure to send it to both
addresses however Section B.4 of the application listed for mailing address must match the NC SOS registration. If the
NC SOS has received a filing to update the mailing address for your company, and they have not yet updated the
webpage, please submit that paperwork to our office as a part of this additional information request.

Application for the subject project on December 5, 2022. A preliminary in-depth review of that information has
determined that the application is not complete and that additional information is needed. To continue the review,
please provide the following additional information in a single, hard copy submittal package:

2. 15A NCAC 02H.1045(3)(a)(ii) or (iii): Please confirm the following:

corporate addresses on file with the NC SOS's corporation database. Section B.4 of the
application is intended for other related addresses and contact information. If additional
addresses are desired, a separate page may be attached.
Please remember to confirm that any revised information is presented consistently throughout the application
documents. Please also remember to provide a hard copy of any updated documents per 15A NCAC
02H.1045(3).
The requested information should be received in this Office by January 10, 2022 in accordance with 15A NCAC

The annual SCM inspection was con�{ucted on the infiltration basin located at the Dollar General
Holden Beach store on February 1, 2022. Upon inspection, the basin appears to be functioning as
designed. Please continue to maintain the vegetation in and around the perimeter of the basin and
makes sure it is kept free of trash and/debris.
The fee for the annual SCM inspection per the Brunswick County Stormwater Ordinance is $75.

Post -Construction Stormwater Management Permit SW8031130 for the subject project expired on April 13, 2018.
This is a reminder that permit renewal applications are due 180 days prior to their expiration per 15A NCAC
2K 1045(3). North Carolina General Statutes and the Coastal Stormwater rules require that this property be
covered under a stormwater permit. Failure to maintain a permit subjects the owner to assessment of civil
penalties. More information about the Post -Construction State Stormwater Program can be found on the

Please submit a complete permit renewal form along with a $505.00 fee and other submittal requirements within
30 days. The permit renewal form can be found under the Post -Construction section of this website or at:

construction-2. Also, it was noted in the file review that a designer's certification that the project was constructed
in accordance with the approved plans as required by the permit has not been submitted. Please include this with

address. Please submit a completed Permit Information Update Form to correct your contact information which is
available on the website provided above under the "New Permits & Permit Modifications" section.
