Dear Ms. Grzyb:
The City of Winston-Salem has evaluated data for the Muddy Creek WWTP, Permit No.
NC0050342, according to the Guidance document for the Reduction of Monitoring
Frequencies in NPDES Permits for Exceptionally Performing Facilities. We request
reduced monitoring for the following parameters: cBOD, TSS,NH3-N and fecal coliform.
Please include this as part of the renewal process. Enclosed you will find data supporting
this request and we have addressed the additional criteria below:
• This facility has not received a civil penalty assessment for permit limit violations
for any of the target parameters during the previous three years.
• Neither the permittee nor any of its employees have been convicted of criminal
violations of the Clean Water Act within the previous five years.
• This facility is not under an SOC.
• This facility is not on EPA's Quarterly Noncompliance Report for target parameter
limit violations.
