This project meets the documentation requirements and approval procedures under NEPA, as defined by FHWA, for Low Impact Bridge Replacements. This project meets the standards of NCDOT's Minimum Criteria Rules. Date: 6/18/15 Applicant Name: NCDOT Division 9 Applicant Address: 375 Silas Creek Parkway Winston-Salem, NC 27127 Primary Contact for Project: Amy Euliss Phone No.:336-747-7802 Is this an after-the-fact application: Yes X No River Basin: Yadkin Pee-Dee Stream Classification: C Regulatory Authorization Options for this Activity Federal: USACE Nationwide General Permit 3— Maintenance State: General Water Quality Certification #3883 Local: None Project Description — This project involves the replacement of Bridge No. 40 on SR-1298 over Swearing Creek in Davidson County. The existing bridge is a 3-span bridge which is 135' long and 30 wide'. The proposed structure is a 3-span bridge which is 150' long and 33' wide. The proposed bridge shall consist of deck drains that will NOT drain directly into the jurisdictional limits of the stream; Class B rip rap pads will be provided under deck drains where slope protection does not cover exposed ground in the floodplain (including the wetland). Permanent impacts have been minimized by adding skew to proposed bridge and by keeping proposed piers mostly out of the jurisdictional limits of the stream. Permanent wetland fill impacts totaling 0.03 acres are for rip rap pads under deck drains for energy dissipation, and new piers. Mechanized clearing impacts totaling 0.06 acres are required for installation of silt fence, and excavation impacts total <0.01 acres are for installation of a rip rap pad to dissipate energy from road stormwater. Temporary wetland impacts of 0.08 acres are required to remove an existing concrete-lined fill face and concte ps. There will be 44 feet of temporary impact on the existing channel Signature: Print Name: /tM U> rn » ( ) liC Title: W ' J,,on 'IransporlaCion Pennliting Unit 1617 Mail Service Center, Raleqh, North Carollna 27699iB50 Lora'on: 512 N Salisbury SI., Raleigh, Norih Caroltnz Phone: 919-707-87841 FAX: 919-733-1250 Internet. hitp:llportal.ncdencorglweblwq An Equal Oppatlanity V AKrma[ive Ailon Enployei Onc NorthCarolina aturallr WETLAND PERMIT IMPACT SUMMARY 8n Existing Exisling Permanent Temp. This bridge replacement project is part of fhe Low Impacl Bridge Replacement Program. The proposed bridge shall consist of deck drains ihat will NOT drain directly into ihe jurisdictional limits of the siream; Class B rip rap pads will be provided under deck drains where slope proteclion does not cover exposed ground in the floodplain. There will be a sormwater drainage system consisting of 2- grate inlets in shoulder berm gutter at the low point of the roadway vertical alignmenl and associated pipe systems. The outlel pipe shall consist of a 15" corrugalled aluminum or steel pipe along the existing roadway emabankment slope (approx. 3:1). A rip rap ouflet pad per NCDOT standard drawings will be placed at the outleL It is not recommended to place a pre-formed scour hole within lhe limits of a jurisdictional wetland. The existing wetland at the outlet will have to be excavaed for the rip rap pad. Adding an elbow at ihe pipe oullet will furiher reduce outlet velocities.'NOTE: Swearing Creek al Ihis location is not on the construction-related 303(d) list of impaired streams.
