On May 14, 2014, NCDOT Division 9 provided you with an Attachment L for the replacement of
Bridge No. 119 on SR 1636 (Buffalo Road) over Big Beaver Island Creek (Ctass C) in Stokes
County.
There is an LJT to Big Beaver Island Creek that NCDOT did not anticipate impacting; however,
during the project final, it became appazent that the toe protection extended below the ordinary
high water mark. The additional permanent impact to this area is 25-linear feet. Total permanent
bank stabilization now equals 60-feet.
By copy of this letter, we also request 401 review and approval from the NC Division of Water
Resources with one copy of this letter and a plan sheet provided to Amy Chapman,
Transportation Permitting Unit Supervisor, and Dave Wanucha, DOT Project Coordinator.
By copy of this letter, we also request Mazla Chambers, NC Wildiife Resources Commission, to
comment directty to the Corps concerning the modification request.

This project is subject to the Low Impact Bridge Process as agreed to on October 6,
2009. This project meets the documentation requirements and approval procedures
under NEPA, as defined by FHWA, for Low Impact Bridge Replacements. This project
meets the standards of NCDOT's Minimum Criteria Rules.

Project Description — NCDOT proposes to replace Bridge no. 119 on Buffalo Road (SR1636)
over Big Beaver Island Creek in Stokes County. There wiil be a total of 70' of rip rap placed
along the banks for stability. Of the 70' of impacts, 35' will be in the same location as the
existing concrete abutment. Therefore, we are requesting 35' are new permanent impacts.
In addition, there will be 20' of temporary impacts due to placement of impervious dike for the
removal of the existing interior bent and rip rap placement. There is 1 UT to Beaver Island
Creek, but there will be no impacts to this channel. The project area is lacking habitat for the
James Spiny Mussel. The project area was surveyed for Roanoke log perch on April 21, but
none were found. (see attached report from NCDOT Biological Surveys). There is no habitat for

the small-anthered bittercress. There is habitat for Schweintz's sunflower, however none were
found during survey for previous year's plants and young plants on April 21, 2014. The project
has been designed to treat post-construction stormwater to the maximum extent practicable.
There will be no deck drains over the water. Stormwater will be collected and discharged at
non-erosive velocities onto an energy dissipater pad in the southwest and northwest quadrants
of the bridge. Erosi control has been designed per NCDOT BMPs.
