Andy, David,

This email contains the PCN and the supporting documents for a portion of project W-5208J (Hwy
401 Reverse Superstreet Project) in Hoke County near Rockfish. This site involves a small area of
wetlands near one of the “bump outs” that was not caught by me during the initial scoping. Due to
a miscommunication with the contractor, the area has already been impacted. Please note that this
is an “after the fact” PCN. Attached to this email are:

1 -set of construction plans
1- wetland data form
1-hand sketch depicting wetland sampling points
1-PCN (word version)
1-scanned PCN with signature (PDF)

The In Lieu Fee request form has already been sent to Ms. Beth Harmon at EEP.
