REGAN Secreary LINDA CULPEPPER in f*6arortw August 29, 2018 KF, NETH EDWARDS — PRESIDENT BASS FARMS, INC POST OFFICE BOX 126 SPRING HOPE, NORTH CAARROLINA 27882 Dear Mr. Edwards: Subject: Permit No. WQ0002004 Bass Farm Sausage WWTF Wastewater Irrigation System Nash County In accordance with your permit renewal and minor modification request received May 9, 2018, and subsequent additional information received June 21, 2018 and August 13, 2018, we are forwarding herewith Permit No. WQ0002004 dated August 29, 2018, to Bass Farms, Inc for the continued operation of the subject wastewater treatment and irrigation facilities. Please note that this renewed permit shall become effective on November 1, 2018 (i.e., the day after the expiration date of the existing permit). The modifications to the subject permit are as follows: irrigation field 3 (the field furthest west on the Bass Farm property) has been removed from the permit. Sprinkler heads and headers are to be decommissioned in accordance with new permit condition 1.1. This permit shall be effective from November 1, 2018 until October 31, 2023, shall void Permit No. WQ0002004 issued July 5, 2013, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements listed in Attachments A and B for they may differ from the previous permit issuance. Failure to establish an adequate system for collecting and maintaining the required operational information shall result in future compliance problems. If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 15013 of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC 27699-6714.
