Failure to submit all required items will necessitate additional processing and review time. For more information, visit the System-wide Collection System Permitting website: General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested additional information. The Applicant shall submit one original and one copy of the application and supporting documentation. The copy may be submitted in digital format. A. Cover Letter ® Submit a cover letter listing all items and attachments included in the permit application package B. No Application Fee Required ➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit > The appropriate annual fee for systemwide wastewater collection system permits may be found at: ➢ Annual Non-Discharge Fees C. System-Wide Wastewater Collection System(FORM: CSA 04-16)Application: l Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04- 16) application. Any unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or due to space restrictions, attachments to the application may be made,as long as the attachments are numbered to correspond to the section and item to which they refer. ❑ If the Applicant Type in Section I.3 is a Privately-Owned Public Utility, provide the Certificate of Public Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the wastewater collection system,or ❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an application for a franchise has been received and that the service area is contiguous to an existing franchised area or that franchise approval is expected. ❑ If the Applicant Type in Section 1.3 is a corporation or company, provide documentation if it is registered for business with the North Carolina Secretary of State. SPILL RESPONSE AND CLEAN-UP PLAN The following document outlines the City of Lexington's Sanitary Sewer Overflow response plan. Its purpose is to establish guide lines and procedures that will ensure prompt and effective action during an S.S.O. All spills must be checked within 2 hours of complaint or notification. REPORTING PROCEDURES After the initial assessment of the situation,the crew leader shall determine if the S.S.O. requires reporting. An S.S.O. must be reported to NC DEQ if: The spill is 1,000 gallons or more; or The spill, regardless of the volume,reaches the surface waters of the state.Please note that conveyances such as ditches and storm sewers that could drain to surface water must be included. If in doubt,the crew leader shall regard the spill as reportable. If the spill requires reporting,the crew leader shall notify one of the following: Maintenance& Construction Superintendent- ORC Nextel-240-2016 Utility Service Crew Supervisor-Backup ORC Nextel-240-2017 Upon determining a sewer spill,the responding employee shall notify the Public Works ORC or Backup ORC immediately to make the required reports to the Winston-Salem Regional Office of the NC Division of Water Resources. This shall include an oral report to Division of Water Resources within 24 hours of first knowledge of the spill,and an electronic SSO report to NC DEQ at the following webpage within 5 calendar days of first knowledge of the incident: https://bimsportal.ncdenr.org/usersandsecurity/pages/bimsLogin.jsp. If the Maintenance &Construction Supt. is unable to make these reports,they will be posted by either the Utility Service Crew Supervisor or the Water Resources Utilities Director. If the reportable S.S.O. occurs on a weekend or holiday, it shall be the responsibility of the highest ranking member of the responding crew to notify the Division of Emergency Management at 1-800-858-0368 or(919) 733-3300 within 24 hours knowledge of the spill. The M&C Supt. (ORC)or Backup ORC shall then follow up with the required written report. If the ORC's are unable to make these reports,they will be posted by the Water Resources Utility Director. (1) In the event of a discharge of 1,000 gallons or more of untreated wastewater to the surface waters of the State,the Water Resources Utilities Directorshall issue a press release to all print and electronic news media that provide general coverage in Davidson County(The Dispatch), setting out the details of the discharge, as provided below. The Water Resources Utilities Director shall issue the press release within 24 hours after the ORC/Backup ORC has determined that the discharge has reached the surface waters of the State. The Water Resources Utilities Director shall retain a copy of the press release and a list of the news media to which it was distributed for at least one year after the discharge and shall provide a copy of the press release and the list of the news media to which it was distributed to any person upon request. Press Release NOTIFICATION OF DISCHARGE OF UNTREATED WASTEWATER General Statute 143-215.1C requires that the owner or operator of any wastewater collection or treatment works to issue a press release when an untreated wastewater discharge of 1,000 gallons or more reaches surface waters. In accordance with that regulation,the following news release has been prepared and issued to The Dispatch in Davidson County: The City of Lexington had a discharge of untreated wastewater on DATE of an estimated # gallons at FACILITY NAME/LOCATION ,near the intersection of CLOSEST STREET INTERSECTION. The untreated wastewater was discharged into NAMED STREAM or UNNAMED TRIBUTARY TO NAMED STREAM in the Yadkin River Basin. The NC Division of Water Resources was notified of the event on DATE and is reviewing the matter. For more information contact the City of Lexington Water Resources Dept. at(336)248-3930. (2) In the event of a discharge of 15,000 gallons or more of untreated wastewater to the surface waters of the State,the Water Resources Utilities Directorshall publish a notice of the discharge in The Dispatch and the Denton Orator and newspapers having general circulation in each county downstream from the point of discharge that is significantly affected by the discharge. The Secretary of the NC DEQ shall determine, at the Secretary's sole discretion,which counties are significantly affected by the discharge and shall approve the form and content of the notice and the newspapers in which the notice is to be published. The notice shall be captioned "NOTICE OF DISCHARGE OF UNTREATED SEWAGE". The Water Resources Utilities Director shall publish the notice within 10 days after the Secretary has determined the counties that are significantly affected by the discharge and approved the form and content of the notice and the newspapers in which the notice is to be published. The Water Resources Utilities Director shall file a copy of the notice and proof of publication with the Department within 30 days after the notice is published. Publication of a notice of discharge under this subdivision is in addition to the requirement to issue a press release in Section(1) above. Public Notice NOTICE OF DISCHARGED OF UNTREATED SEWAGE The City of Lexington had a discharge of untreated wastewater from our wastewater collection system at FACILITY NAME/LOCATION at the intersection CLOSEST INTERSECTING STREETS of approximately # gallons. The discharge occurred on DATE forapproximately # hours. The untreated wastewater entered into NAMED STREAM or UNNAMED TRIBUTARY TO NAMED STREAM in the Yadkin RiverBasin. The FACILITY NAME/LOCATION was repaired and placed back into service by ACTIONS TAKEN TO CORRECT PROBLEM to prevent further discharge. CORRECTIVE MEASURES The first objective of the responding crew to anyS.S.O. is to correct the problem and contain the spill as soon as P g possible. The highest ranking member on the crew shall be responsible for determining the procedures required and has the authority to procure the equipment needed to make repairs as quickly and safely as possible. Spare parts for pumps,replacement pumps and repair materials will be available at the City Warehouse at 425 Carolina Ave. Any additional materials or equipment needed can be purchase using the City of Lexington purchasing card. If the problems associated with the S.S.O. cannot be corrected within four hours,the highest ranking member on the crew shall notify the Superintendent or the Water Resources Utilities Director. CLEAN UP PROCEDURES Containment of the spill shall be the prime objective of the responding crew. If possible,any overflows should be damned to prevent contamination of surface waters. The dam shall be constructed of soil that is capable of holding the spill plus water used to clean or flush a contaminated area. If the area is left unattended,warning tape shall be placed around the perimeter. Stock piles of lime,straw,and sand shall be available for use during clean up. If the spill cannot be adequately cleaned by in house crews,or presents a serious immediate threat to public health or safety, a professional hazardous waste handler shall be called in. Road Ways with Storm Drainage or Ditches 1. Set up containment to prevent spills from entering ditch or drain 2. Spread sand to soak up spill 3.
