However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 1, 2023. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through H, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Some of the cumulative pollutant loading rate (CPLR) forms provided are dated as early as 2013 (such as for field NC-CS-10-04), while other fields did not have a CPLR submitted (such as for field NC-CS-10-04). Please provide the most recent CPLR for all fields in the application and clarify whether the submitted forms represent the most recent land application events or if land application has never been conducted on a field. 2. Some fields are proposed to be broken out into separate fields (like NC-CS-06-02 turning into CS- 09B and 09-C) or combined to create a single field (such as NC-CS-06-03 and 04). Please provide an explanation for how the CPLR will be accounted for in these changes. 3. The predominant soil series listed in the LASC summary page were different than the listed series in the previous permit for most of the renewed fields. Please clarify why these are different and provide a justification for why the predominant soil listings should be changed if that is what is being proposed. 4. Fields NC-CS-06-03 and NC-CS-06-04 are proposed to be combined into the new field NC-CS- 06-03, but these fields were previously permitted with different dominant soil series. Please provide a justification that these fields can be combined and managed under the proposed soil series. 5. Sections of the previously permitted field NC-CS-07-01 are proposed to be included in field NC- CS-07-02 instead. These fields previously had different soil series and are shown to have different soil series in the provided LASC summary page. Please provide a justification that the areas removed from Field 01 are consistent with the predominant soil series of Field 02. Mr. Alan Johnson May 2, 2023 Page 3 of 3 6. Based on the comparison between the previously approved site maps and the proposed site maps, the following fields appear to include areas that were not formerly permitted: a. NC-CS-06-01. An area in the southwest corner of the field previously had two ditch features buffered out, but these features are absent in the new maps. b. NC-CS-09-01. There were water and ditch areas buffered out on the northern areas of the field which extended further into the field than what is currently proposed. Additionally, there are new areas in the northeastern most portion of the field that were not previously included in the footprint of the field that are included in the proposed maps. c. NC-CS-09-03. The northern portion of the field which previously had areas buffered out extending further into the field than what is currently proposed. The new land application area extends up to the property line setback. d. NC-CS-12-05. A tree stand buffered out of the previous map that is now cleared and included. e. NC-CS-14-01. A feature in the middle of the site which previously bisected the field which is no longer depicted. f. NC-CS-14-02. The northwestern portions of this field which was previously buffered out appear to be included in the new map. These areas shall be certified by a Licensed Soil Scientist (LSS) to verify that they are suitable for land application. An abbreviated soils evaluation in accordance with Section B of the Instruction for FORM: LASC 06-16 shall be submitted to verify that these sections of each field are consistent with the soil series currently listed in the permit and suitable for land application. This shall be signed and sealed by an LSS. G. Landowner Agreement (FORM: LOA 06-16) Form: The provided LOA for NC-CS-07-02 signed by Gregory Todd Hodges did not include the Permittee's Certification. This form shall be executed and re -submitted for review. H. County Board Notification (FORM: CBN 06-16): 1. No comments.
