Dear Mr. Belk:
On April 20th, 2023, Division of Water Resources staff Ron Boone conducted a routine
compliance inspection at Holden Crossing Shopping Center Wastewater Collection System.
Wendy Vaughn was present during the inspection. A review of records and the collection
system was completed. This review reflected compliance with the subject permit.
The following violations were noted:
None
The following items of concern were noted:
None
Other observations:
1. Ms. Vaughn presented a system map that showed all the required information.
2. Ms. Vaughn presented an acceptable operation and maintenance plan, which included
pump station inspection frequency.
3. The pump station is equipped with audible/visual alarms as well as a telemetry
communication system and is inspected a minimum of once per week by a contractor.
4. There are no high -priority sewers in the system.
5. There are no right-of-ways in the system.
6. Overflows must be reported to the Winston-Salem Regional Office at 336.776.9800
within 24 hours.
7. There are several grease traps in the system. BVB Properties works with each of the
tenants that have a trap to maintain it properly and have it pumped out as needed.

Inspection Summary:
Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office (WSRO), conducted a
compliance evaluation inspection of the Holden Crossing Shopping Center's wastewater collection system on April 20th,
2023. Wendy Vaughn, property manager, was present for the inspection. The Holden Crossing Shopping Center's collection
system is deemed permitted. Findings of the inspection are summarized below:
OBSERVATIONS:
(1) Ms. Vaughn presented a system map that showed all the required information.
(2) Ms. Vaughn presented an acceptable operation and maintenance plan, which included pump station inspection
frequency.
(3) The pump station is equipped with audible/visual alarms as well as a telemetry communication sytem and is inspected a
minimum of once per week by a contractor.
(4) There are no high -priority sewers in the system.
(5) There are no right-of-ways in the system.

Please be aware that failure to fully comply with applicable regulations violates North Carolina General Statute (NCGS)
143-215.1 and NCGS 143-215.6A allows a civil penalty of not more than twenty-five thousand dollars ($25,000) against any
person who violates 143-215.1.
