The purpose of this letter is to respond to your letter for Permit # WQCS00350 dated 1-25-21 and
received on 2-1-21. Your letter requested that we address the causes of the SSO, remedial actions, and all
other actions taken to prevent the recurrence of this SSO.
Causes: Hurricane Zeta created a power outage, the automatic transfer switch functioned correctly and
we ran on generator power. The generator ran until one of the two belts broke. This generator is
exercised weekly and it was just bad luck that the next time it ran we had belt failure.
Remedial Actions: we had no generator replacement belts at that time for this generator, so we got a
closed business to open for us to get a standard belt. The difficult belt replacement was performed and
we got back on generator power. The generator ran again until the other belt broke. We then got the
generator company to supply us with a matching belt set for generators and the difficult belt
replacement was performed again.
Prevent recurrence actions: we now have additional matching generator belts on hand at our
maintenance shop. In the future, both belts will always be replaced at the same time and on a
schedule.
