Snider, Regional Supervisor Water Quality Regional Operations Section Winston-Salem Regional Office Division of Water Resources, NC DEQ Subject: Notice of Violation & intent to Issue Civil Penalty Tracking No: NOD-2021-DV-0062 Sanitary Sewer Overflows —July, 2021 Collection System Permit No. WQCS00018 Eden Collection System Rockingham County Dear Mr. Snider, NC Department of Environmental d Quality Recei OCT 14 2021 Winston-Salem ,J Regional Office This letter is in response to a Notice of Intent to Issue Civil Penalty that has been issued against the City of Eden Collection System Permit No. WQCS00018 as of October 4, 2021 for incident number 202101524. Incident number 202101524 concerns a sanitary sewer overflow that occurred on July 8, 2021 from the force main on an outfall approximately 200 foot from Edgewood Dr. Approximately 300 gallons of untreated sewage was released with 150 gallons reaching unnamed tributary leading to the clan river. The cause of the overflow was 1/1 finding its way into the sanitary sewer outfall line down during a rain event because of damage done to a manhole on an outfall by what appeared to be a truck knocking off the ring and cover, allowing a drainage ditch to dump directly into the manhole. That particular line is being evaluated for replacement in the near future. The City of Eden takes sanitary sewer overflows seriously and is doing everything it can to eliminate them. Please take these facts into consideration when deciding what action to take. Sincerely, 4 y Mike T. Vernon O.R.C. City of Eden Collection System I&I Foreman City of Eden CC`.
