This letter is in response to the Notice of Intent, Case No NOV-2021 -DV-01 39 concerning the spill that
occurred on November 13'", 2020 at 3802 Samet Drive Guilford County, High Point, North Carolina.
This above spill was due to debris in the line which occurs during tropical storm Eta. During this event
there was up to 5 inches of rain that fell within a 24-hour period. The amount of flow that was forced
into the system caused a step in the manhole to break off and fall into the invert causing a partial
blockage. The step was removed, and all manholes in this area were inspected to ensure that they
were free of any other debris. This location is being reviewed by our engineering department to help
with flooding and other issues to create a plan to prevent any future spills in this area. We do believe
this was an isolated incident due to the cause. The area was cleaned, once the water receded and
since the event, we haven't had any other issues.
