This letter is in response to the Notice of Intent Case No. NOV-2021-DV-0058 concerning the spill that
occurred on June 301", 2021 at 1300 Prospect Street, Guilford County, High Point, North Carolina.
The overflow that occurred at the above address was caused by shop towels/hand wipes. Once the
Vac -All trucks were onsite they began to work on the blockage as well as vacuuming up any flow that
reached the ground to ensure that it wouldn't make it to surface waters. Once the manhole dropped the
surrounding area was cleaned and limed. Afterwards inspections were performed upstream and found
that the debris came from a commercial industrial user upstream that performed cleaning that morning.
This line was cleaned and flushed and will be periodically inspected and added to our prevented
maintenance to help prevent any future spills in this area, but we believe this was an isolated incident
since there is no prior records of issues at this location. We haven't had any issues on this line since
the overflow.
