Snider, Regional Supervisor North Carolina Department of Environmental Quality Division of Water Resources 450 West Hanes Mill Road, Suite 300 Winston-Salem, North Carolina 27105 Re: Sanitary Sewer Overflow Civil Penalties Incident #202101444 Dear Mr. Snider: NC Department of Environmental Quality Received SEP 2 9 2021 Winston-Salem , Regional Office This letter serves as the official response from Winston-Salem/Forsyth County Utilities (WSFC Utilities) to the North Carolina Department of Environmental Quality, Division of Water Resources Notice of Intent to Enforce Civil Penalty for the June 23, 2021 sanitary sewer overflow (SSO), Incident #202101444, located across from 1045 Industrial Park Drive. The incident was a result of pipe failure due to severe natural conditions. On June 23, 2021 at 1:30pm, an SSO occurred in the outfall across from 1045 Industrial Park Drive. Our easement clearing crew was in the process of completing routine easement maintenance when they came upon a 10-inch pipe crossing a ditch that had failed due to recent heavy rains. Responders continued work with clearing equipment so the job could be accessed by the repair crew. The easement crew cleared the easement and the pipe was temporarily repaired by the construction crew with a new section of pipe. Normal flow resumed within six hours. Crews cleaned the site with a backhoe and skid steer. A diffuser was deployed for 24 hours and the spill area was cleaned following standard protocol. Upstream and downstream mains were cleaned and inspected by CCTV with no visual defects observed. This line has been added to our current projects and will be permanently repaired with ductile iron pipe. The embankments will also be restored to provide a more secure foundation for the pipe in the future. City Council: Mayor Allen Joints; Denise D.
