Inspection Summary:
On April 19, 2023, Division of Water Resources staff Caitlin Caudle conducted a review of the 2022 Annual Report. This
review reflected compliance with the subject permit.
Other observations:
• 721.24 tons were applied, requiring quarterly sampling. Sampling was completed on 3/16/2022, 4/20/2022, 5/4/2022,
and 12/29/2022. Application events occurred in March, April, and May.
• Selenium was misreported on the Annual Residual Sampling Summary Form for the 12/29/2022 sampling event. Lead is
listed as 1670 mg/kg on the form and <9.14 mg/kg on the analysis report.
• The Annual Land Application Summary form reported 23 fields currently in the permit. According to the most recent
Attachment B issued September 14, 2022, there are only 10 fields in this permit.
• Six fields were utilized in 2022.
• Field DV-09-02 and RO-01-01 both had pH < 6.0. Neither field was reported to have received lime applications in 2022.
• 30-day bench scale test and volatile solids reduction were used as the vector reduction method.
