Dear Mr.Parker,
Carolina Water Service,Inc.of North Carolina("CWSNC")and the Town of Dallas entered into an Agreement for
Wholesale Bulk Wastewater Treatment on May 14,2014 for the Town of Dallas to provide wastewater service for
our College Park community. CWSNC retains the collection system and customers and transfers the bulk
wastewater to Dallas for treatment. A gravity sewer extension and interconnection between our systems was
permitted under WQ0037213 which is now complete.
The College Park wastewater treatment plant has been off-line and has not been discharging effluent as of August
13,2014 when the wastewater starting flowing to the Town and our facility has now been decommissioned.
Therefore,we hereby voluntarily and officially request that the NPDES permit be rescinded effective immediately.

Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
This final permit includes the following change from the draft permit sent to you on December 2, 2009.
• Parameter codes for BOD,TSS and Ammonia Nitrogen have been restored to those existing in the
current permit. We apologize for any inconvenience this may have caused.

If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh,North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

PART IV-EVALUATION AND RECOMMENDATIONS
The Permittee,Utilities,Inc.,previously Carolina Water Services,is applying for renewal of
the facility's NPDES permit to discharge treated domestic wastewater. The WWTP appeared to be
well maintained and operated. No problems were noted during the investigation.
Please note that there is no post aeration at the facility. Please remove it from the description
page and add the de-chlorination system.
Pending review and approval by the Western NPDES Program Unit,it is recommended that
the permit be renewed as requested.
