Robbins, Owner 2523 Nealwood Ave Graham, NC 27253 NORTH CAROLINA Environmental Quality April 4, 2023 Subject: Follow -Up Compliance Evaluation Inspection Single Family Residence Wastewater Treatment System 2523 Nealwood Ave, Graham, NC 27253 General NPDES Wastewater Permit: NCG550000 Certificate of Coverage: NCG550263 Alamance County Dear Mr. Robbins: Ron Boone, of the NC Division of Water Resources (DWR), Winston- Salem Regional Office WSRO), performed a follow-up inspection of your single family residence wastewater treatment system on March 20th, 2023. This was a follow-up to the inspection he performed on February 15th, 2023. Mr. Boone noted the following findings from the follow-up inspection: 1. You purchased the home in January, 2021. 2. You understood the home had a traditional septic system with subsurface drainage field when you purchased the home, rather than a discharging system with a subsurface sand filter, chlorinator, and discharge pipe. 3. You are unfamiliar with the wastewater treatment system at your home, and the fact that it has a subsurface sand filter, chlorinator, and discharge pipe into the stream. 4. You do know the approximate location of the septic tank and you have not had it pumped yet. 5. The chlorinator was located during the inspection. It consists of two 4" white PVC capped pipes that protrude approximately 12" from the ground surface in the front yard approximately 40 to 50 feet immediately south of the large tree. There were no tablets in the chlorinator at the time of the follow-up inspection. 6. You were unaware of the discharge permit (NCG500000) and certificate of coverage (NCG550263). 7. The certificate of coverage is still in the previous owner's (Elton Cafferata) name. 8. You stated you still receive mail for the previous owner of the residence and normallyjust throw it away. This explains what happened to the previous three overdue annual fee invoices. 9. The last three annual fees for the general permit/certificate of coverage are unpaid and overdue. Mr. Boone has emailed you with important information regarding the proper chlorine to use in the system as well as a Name/Ownership Change (NOC) form. Please submit the NOC form as directed and submit payment of the annual fees. You are reminded the general permit requires annual testing of the treatment system's effluent. If the effluent pipe is found, samples of the discharge should be taken for analysis in accordance with the permit. If you experience issues with the home's plumbing system, it may be due to improper operation and maintenance of the treatment system and/or the missing discharge pipe. Efforts should be made to restore the entire system to proper operation or attain a permit from the county health department and have a traditional septic system installed to replace the existing discharging system, at which point this permit may be rescinded. Pursuant to NC General Statute (NCGS) 143- 215. 6A, a civil penalty of not more than twenty-five thousand dollars ($ 25, 000.00) may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S. 143-215. 1.
