Snider NCDENR Winston-Salem Regional Office 450 West Hanes Mill Road, Suite 300 Winston-Salem, NC 27105 RE: Request for Remission Notice of Violation Case # NOV-2020-DV-0015 Incident Number 201901592 09/25/2019 Mr. Snider, NORTH CAROLINA'S INTERNATIONAL CITY' NC Department of Environmental uality Recei FEB 28 2020 Winston-Salem Regional Office This letter is in response to the Notice of Violation and Assessment of Civil Penalty, Case # NOV-2020- DV-0015, concerning the spill at the Intersection of Whites Mill Road. and Skeet Club Road., Guilford County, High Point, NC. The spill at the above -mentioned address lasted for 30 minutes. The spill was caused by a NCDOT contractor tying in a new force main when a plug blew out causing the spill. The force main was shut down and clean up was performed by the contractor. The City of High Point was notified of the spill the next day when Ms. Patty Eason with NCDOT called asking for our report of the incident. I informed Ms. Eason the City was unaware of any spills as none had been reported to us. Ms. Eason informed me that NCDOT had gathered information about the spill from the Environmental Specialist and engineers on site. NCDOT's onsite personnel had taken pictures and calculated the spill volume while the contractor performed the repairs and clean-up by washing the spill debris into a storm water catch basin. After being notified on 9/26/19, 1 went to the site to see if any evidence of the spill was still present and where the spill had flowed to. It was determined that all discharged waste had been flushed to a tributary leading to West Deep Fork River (Oak Hollow Reservoir). The reservoir was monitored for adverse effects. A Press release was issued for the event. The work was completed on the force main the same day as the spill and put back in service. The City of High Point first received notification of a Sanitary Sewer Overflow (SSO) on 09/26/19 at 11:47 AM. The ORC of the collection system was advised by the City of High Point Street Maintenance Division that an SSO had occurred on 09/25/19 at the Intersection of Whites Mill Road and Skeet Club Road and that NCDOT had contacted the Street Maintenance Division about needing information on the SSO that occurred the previous day. The ORC was informed by the Street Maintenance Division that a NCDOT contractor doing work to the sanitary sewer force main for NCDOT had a plug blow out causing the sewer overflow. Around 12:00 pm on 09/26/19 the ORC and Back -Up ORC visited the site to review the spill information, to see if there was debris that needed to be cleaned up at the SSO site, and to trace l the SSO downstream to see if any of the liquid could be captured and cleaned up. Patty P. Eason, NCDOT Division Constructiorf Engineer, was contacted aFound 3:00 pm on O9/26/19 to gather information about the SSO such as start time, duration and spill amount. Mrs. Eason indicated their personnel onsite had directed the contractor to flush the overflow material off the roadway. Mrs. Eason also stated that she could provide pictures if it would help. i The SSO was reported to the North Carolina Department of Environmental Quality by Ronnie Massey to Shuying Wang at 3:30pm on 09/26/19.
