Phillips, Vice President 639 Old US 52 South Mount Airy, NC 27030 NORTH CAROLINA Environmental Quality March 1st, 2023 Subject: Follow -Up Compliance Evaluation Inspection NC General Wastewater Permit NCG520000 Slate Sand Mining, LLC Site Certificate of Coverage #NCG520061 Surry County Dear Mr. Phillips: Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office (WSRO), contacted you through email on February 7th, 2023, as a follow up compliance evaluation inspection to the previous inspection conducted in January, 2022, for the Slate Sand Mining, LLC Site located on John Mickles Road in Surry County. The inspection findings are summarized below and in the checklist on the following pages. This is a follow up inspection of the January 2022 inspection. Violations noted during the January, 2022 inspection were: 1.) No signs on the river as required by the permit and 2.) No analytical monitoring had been accomplished. According to you, these violations have now been corrected by installing the required signage and conducting the permit required analytical monitoring. Mr. Phillips sent Mr. Boone photographic evidence of the signs, as well as a copy of the most recent analytical monitoring result reports. Results were satisfactory and compliant. Please be aware that the Division may take enforcement action against any permittee who fails to comply with any requirement of their permit.
